All newly hired employees must have participated with a payroll deduction <br />the entire period to be eligible for a refund. Any employee who cha.nges <br />from a single to a family plan status or vice-versa during the above <br />period shall be entitled to a refund calculated only by the number of <br />months for which contribution was made. <br />If the average cost or premitsn cost per month exceed $525.00 per month, <br />the additional amount sha.ll be paid at a ratio of three-quarters (75%) by <br />the City and one-qua,rter (25%) by the employee up to $650.00. If the <br />average cost exceeds $650.01, that additional amount shall be paid by the <br />Fmployer at a ratio of one hundred percent (100%). The employee share of <br />the cost which exceeds an average of $525.00, shall be withheld as an <br />additional payroll deduction within thirty (30) days following the <br />determination of the actual average cost. <br />Section 24.06. The Enployer will make an earnest effort to establish <br />a committee consisting of one (1) representative from each of the City <br />bargaining units, if they choose to be represented and a minimtun of one <br />(1) representative of the Fmployer. This ba.rgaining unit's role as a <br />participant in the aforementioned committee shall be advisory only. The <br />Employer reserves the right to establish a11 enrollment periods and <br />regulations. <br />A7-1 changes affecting coverage or chosen providers shall be shared with <br />the ba,rgauLing unit prior to the making of such changes. <br />Section 24.07. The Employer shall pay the full amount of dental <br />insurance for all full-time employees and dependents. The features of the <br />plan will be consistent and similar to those in effect on December l, <br />1991. Some of its features being a$25.00 per-person per-calendar year <br />deductible together with an annua,l maximtun of one thousand dollars <br />($1,000) payable per covered person during any benefit year. Those <br />employees an a family health plan will be covered under a family dental <br />plan. Those an a single health plan will be provided with a single dental <br />plan. <br />Section 24.08. The Employer shall pay the full amount of vision <br />insurance for all full-time employees and dependents if employee„chooses a <br />family plan. The features of the plan will be similar and consistent with <br />those in effect Decer,iber l, 1991. Those employees on a family health plan <br />will be covered tmder a family vision plan. Those on a single health plan <br />will be provided with a single vision plan. <br />Section 24.09. Expected Cost or Premium is defined as the cost of <br />all medical, hospital, prescription drug cost, and related fees. <br />Retro-activity shall apply to the terms of Sections 24.01, 24.02 and <br />24.03 of the Agreement. <br />The Finployer reserves the right to establish a11 enrollment periods and <br />regulations. <br />-21-
