a. Research and procure all appropriate records and perform all field surveys and <br />constrvction staking to establish for the contractor the horizontal and vertical <br />control and alignment for the proposed improvements. Said records shall <br />include the improvement plans prepazed by Polytech Inc. <br />b. Maintain a record of all pertinent information regazding the construction <br />at the site with respect to installation or other conditions relevant to the <br />preparation of as-built plans. <br />c. Record on a set of plans all relevant information for establishing an "As-Built" <br />set of plans for all underground structures, sanitary sewers, storm sewers and <br />water piping. Said As-Built plans to be delivered to the City of North Olmsted <br />at project completion. <br />You may submit the price proposal in the format you deem most appropriate; however, the City <br />prefers the Proposal to be an hourly rate for vazious personnel or crews. You may also include <br />any standard terms and conditions of your Proposals, rates for supplementary services, and any <br />clarifications or qualifications to the requested Proposal which you deem appropriate. <br />It is anticipated that the contract for construction will be entered into during August or <br />September, 1996 subject to approval of the Ohio Public Works Commission. Construction is <br />expected to take approximately nine months. <br />Please submit the Proposal not later than July 12, 1996 and direct your Proposal to the attention <br />of the Service Director, Ralph H. Bohlmann, for acceptance by the Mayor.
