Patel, P.E. <br />G & T Associates, Inc. <br />11925 Pearl Road, Suite 401 <br />Strongsville, Ohio 44136 <br />Re: Request for Proposal <br />For Surveying Services for Clague Park Improvement <br />Pavement and Drainage Improvements <br />Dear Mr. Patel: <br />Ed Boyle, Mayor <br />Phone: (216) 777-8000 <br />I am requesting a Proposal to provide professional surveying services for the construction of <br />sanitary sewer, grading, drainage, storm sewer and other improvements at Clague Park at North <br />Olmsted. <br />The Proposal shall include the following items in the Scope of Services: <br />1. General Conditions: <br />a. Work shall not be subcontracted without prior consent of the City in writing. <br />Since this project is partially funded by a grant &om the Ohio Public Works <br />Commission, at least 2 percent of the services contract may be required to be <br />subcontracted to a state certified MBE, which is also approved by the City. <br />b. Additional billable services shall not be performed without prior consent of <br />the City in writing. . <br />c. Key personnel to perform services shall be listed in the proposal. <br />d. The Consultant shall indemnify and hold the City harmless from all losses <br />and damages resulting from the Consultant's failure to meet the standard of <br />care for services at the level customary for competent and prudent engineers <br />and surveyors performing such services. The Consultant shall be responsible <br />for the professional quality, technical accuracy, timely completion and <br />coordination of all services. <br />e. The scope of services may be amended by mutual agreement. <br />f. If a dispute arises between the Consultant and the City regarding the conttact <br />for services, the City and the Consultant shall negotiate in an attempt to resolve <br />the dispute. Should negotiations be unsuccessful, a mutually acceptable third <br />party may be engaged to mediate the dispute, with expenses equally shared <br />between the City and the Consultant. <br />.??n
