BE IT ORDAINED BY THE COUNCI3. OF THE CITY t}F NORTH: OISTED, STATE OF OHIO: Section PERMIT ~BQUIRED, No person, firm or corporation shall co~amence t® ill ~ well for ail or gas within the corp<rra.te limits of the City of N±erth Olt~sted, except pursuant to a permf.t issued by the Building Commissioner of the City of North Olmsted, Ohio, as hereinafter provided. Each i~rell shall require a separate permit. S~ct,,,~'e,,n 2 APPLICATION. Any person, firm or corporation desiring tQ er~gagn the drilling of a well for oil or gas, or to carry on the operation of an oil or gas well within the corporate limits of the City of N®rth Olmsted,. shall make applicatiog to the Building C' ' ssioner of the City of North Olmsted upmn such forms as he ma.T prescribe for a permit to do so. Such application shall be scccaaspamied by a fee of One Hundred (.$100.00) dollars. There shall be filed a sep-» orate application and fee for each well contemplated. Secti®n 3. MAP REQUIRED., It shall be the duty of person, firm or eorp®rati®~ -ewr~ing or leasing property for the purpose of drill- ing for oil or gas thereon, to provide a map or blue-print at the time of making the application, showing the exact location where-the proposed. well is to be drilled and in the event that said perffiit sh®uld be granted, the map or blue-print shall remain on file in the office of the Building Commissioner for future reference. Section INVESTIGATION. The Building Commissioner shall refer each appl.~en .for a permit to the Director of Public Safety who shall cause an investigation to be made . of the pre~,.ses f®r which such permit is sought. When such an investigation is completed the application with the report of the investigation shall be subm~tted to the Planning Commission of the City of North ~?]aasted, and if found to comply in all respects with the terms of this ordinance! they, the Plasaing Commission, may recommend the granting thereof;, otherwise it shall recommend rejection of such application. Section ~, ISSUANCE OF PERMIT.. The Building Commissioner shall issue~e permit applied for, upon receipt of the application du1.y approved by the Planning Gommi.ssion of the City of North Olmsted and upon the furnishing of a bond by the applicant in the sum of One Hun- dred Thousand Dollars 0100,000.00) in form and with surety approved by the Mayor of the City of North O~sted, conditioned upon the faith- ful observance of all provisions of law and ordinance relating tQ drill- ing of an oil or gas well, and further conditioned that the permitee and. his or her assigns will restore the streets and sidewalks and all other public places of the city which may be disturbed in the operation„ to their former condition will clear the area of all litter, machinery, '~, derricks, buildings, oil or substances used in the drilling or production ~- operation and that he or it will pay any and all damages suffered by az:y person or corporation from fire, oil, gas or water damage to property in the City aver and above th® insurance collected thereon caused by or originating from the operations connected with such well and will .hold the City free and harmless from any and all liability growing out of the granting of such permit. Section 6~ DUR.~TIaN OF BOLD. The bond as provided for in Section 5 s~ha~be in force and effect during the entire drilling oper- ation and until such time as the well is capped and far 30 days there- after, at which time, the applicant may discontinue the bond if he has complied with all the conditions thereof, but, in the event that the applicant chooses to discontinue his bond as herein provided, tae must deposit with the Building Commissioner, in lieu of the One Hundred Thousand:(100,000.00) Dollar bond, his insurance policies or certifi- cates of insurance covering liability for property damage and for
