OBJECTIVE <br /> The intent of this Request for Proposal (RFP) is to establish a term contract, in accordance with <br /> the city's codified ordinance section 111.01, with one qualified firm to provide a replacement <br /> roof to the City of North Olmsted. <br /> The City of North Olmsted's Blower Building has aged beyond the reasonable service life and is <br /> no longer subject to effective repairs; as such it is in need of a full tear-off and replacement Due <br /> to the electrical equipment inside and the current condition of the blower building roof it would <br /> be preferred to have the roof replaced as soon as possible during the year 2015. The WWTP is <br /> located at 23775 Mastick Road, North Olmsted, Ohio 44070, the blower building, aka building <br /> 12 is located in the center of the treatment plant, with an exterior aluminum staircase providing <br /> roof access. The City of North Olmsted seeks a qualified contractor to provide a new roof for <br /> the blower building. <br /> SCOPE <br /> 1. Cover Letter- The cover letter shall include: <br /> a. A brief statement of intent to perform the services proposed, <br /> b. Full contact information for the individual designated as the City of North Olmsted <br /> contact on this RFP. <br /> c. Acknowledgement receipt of all addenda issued. <br /> d. Expressly state that, should the Proposer's proposal be accepted, the Proposer agrees <br /> to enter into a contract under the terms and conditions as set forth herein. <br /> e. Proposals without cover letters will be rejected. <br /> 2. Table of Contents -The table of contents shall identify the contents of the proposal in a <br /> format consistent with the proposal requirements and format set forth herein. <br /> 3. Exceptions and Confidential Information -Any and all exceptions to the RFP must be listed <br /> on an item-by-item basis and cross-referenced with the RFP document. If there are no <br /> exceptions, bidder must expressly state that no exceptions are taken. Please Note: Taking <br /> exceptions to proposal requirements may render a bidder's proposal non-responsive and <br /> rejected from further consideration. <br /> Provide a listing by proposal page number of all information, if any, identified as confidential <br /> and the justification for which the bidder deems this information confidential. Please be <br /> advised that a blanket confidentiality or proprietary legend identifying the entire proposal as <br /> confidential or proposals that have had every page identified as confidential will not be <br /> accepted and may cause the proposal to be rejected. <br /> 4. The work scope for this program will include: <br /> a. Removal of the existing debris and gravel located on top of the current roofing material. <br /> b. Removal of the previous roofing material (tear off) from both the roof and the parapet <br /> wall. <br /> c. Assessment of the underlying decking/gypsum board/roof support material <br /> d. Replacement of the underlying roof support material (down to holo-core concrete). <br /> e.
