CCBH 2015 SEMI-ANNUAL REPORT City of North Olmsted Page 5 <br /> REPORTABLE DISEASE SURVEILLANCE/PUBLIC HEALTH INFRASTRUCTURE <br /> GRANT <br /> The Public Health Emergency Preparedness Grant (PHEP) encompasses reportable <br /> disease and surveillance activities. The purpose of this program is to increase public <br /> health response and capacity regarding infectious disease and other illness clusters, <br /> acts of bioterrorism and other major public health events. Activities also include <br /> developing emergency plans and supporting the County Emergency Operations Plan <br /> by serving as the lead agency under Emergency Suport Function (ESF) #8. It assesses immunization status in defined geographic areas to identify <br /> pockets of low immunization. It provides immunization education to physicians and <br /> their staff. Additionally, it helps assure that vaccines are widely available and <br /> affordable. <br /> Program.Inquiries.and Respanses.(in hours)* 81 <br /> Case.Management 1 <br /> CHILDREN AND FAMILY HEALTH SERVICES (CFHS) <br /> This program is designed as an organized community effort to improve the health <br /> status of women and children in Cuyahoga County, including assessment and <br /> monitoring of maternal and child health indicators at the individual community or <br /> neighborhood level.
