OBJECTIVE <br />The intent of this Request for Proposal (RFP) is to establish a contract, in accordance with ordinance <br />section — with an experienced, qualified roofer for the purpose of repairing and replacing the <br />roofing system on NOWWTP 15t floor section of building #14: The lab roof. <br />The City of North Olmsted seeks a qualified contractor ("Provider/Roofer") to provide: tear off, <br />remove, and repair of existing roof penetrations, leveling and installation of a Gypsum -Fiber <br />Roof Board, and installation of a torch applied base and cap sheet. <br />Outline <br />The City of North Olmsted operates a wastewater treatment plant at 23775 Mastick Road, North <br />Olmsted Ohio under the OEPA (Ohio Environmental Protection Agency's) NPDES (National Pollution <br />Discharge Elimination System). The WWTP has recently completed construction of several new <br />buildings as well as modifications to existing structures. Building #14: The lab building consist of several <br />floors, including the upper most roof which and a lower roof about the laboratory building. This section <br />of the plant dates back to 1959 and has been reroofed on several occasions. The existing roof consists <br />of a concrete deck, expanded polystyrene, wood fiber board and or foam insulation, and a black rubber <br />EPDM roofing material. The roof is not currently leaking but has been patched extensively. There is also <br />an unconventional rubber flashing between building 14 and 15 that will need to be addressed/replaced <br />as part of this project. <br />SCOPE OF WORK <br />1. The work scope for this program will include: <br />i. 1. Remove and properly dispose of existing coping metal, EPDM roofing material, wood fiber <br />board, and expanded polystyrene down to concrete deck. <br />ii. Prepare surface and install new roofing surface in accordance with attached specifications. <br />iii. Replace existing, unconventional flashing between building 14 and 15. <br />iv. Recondition existing drains. <br />V. Repair or Install flashings around all other deck penetrations. <br />vi. Repair/eliminate old/unused roof penetrations <br />vii. Encapsulate parapet walls. <br />viii. Dispose of all trash and job related debris in accordance with federal, state, and local <br />regulations. <br />ix. Provide a 20 year warranty. <br />X. Follow specifications as provided in Attachment 1: Hazen and Sawyer, Engineers for the City <br />of North Olmsted WWTP Improvement Project. <br />2. Cover Letter — Each bid shall include a cover letter stating: <br />a.
