Cuyahoga County Board of Health Programs and Services <br />The Cuyahoga County Board of Health (CCBH) of a wide range ofpublic health services to the citizens and <br />communities in our health district. Some of these services may, require income qualification or other qualifying <br />criteria. The focus of some of our grant related activities may, be specific, based on guidance from our funding <br />agencies. The following is a list of services. For more information, please call (216) 201-2000 or visit us on <br />the web at www. ccbh. net. <br />Community and Family Services <br />IMMUNIZATION CLINICS <br />CCBH Immunization Clinics provide immunizations for all ages and TB testing. Travel vaccine clinics are <br />available twice per month. Other efforts include community outreach flu clinics, immunization clinics at refuge <br />centers, and immunization clinics at long term care facilities. <br />NEWBORN HOME VISITING: WELCOME HOME <br />Mothers that are 200% or greater of the Federal Poverty Level are eligible to receive home visits from a public <br />health nurse. Also eligible for a visit are all teen moms and any mom with an identified medical or social risk <br />factor. The nurse will discuss basic newborn care, home safety, parenting, feeding, and concerns of the <br />parent(s). Additionally, the visit will consist of an assessment of the baby, and will provide the parent(s) with <br />resources and make referrals to community agencies as needed. <br />LEAD POISONING PREVENTION <br />Public health nurses provide lead screening, education, and case management. Public health sanitarians <br />perform investigations in response to reports of elevated blood lead levels in children to identify potential <br />sources of lead exposure. Lead risk assessments, housing remediation, and clearance testing are conducted as <br />part of collaboration with the Cuyahoga County Department of Development and our municipalities, through a <br />grant with the U.S. Department of Housing & Urban Development. <br />CHILDREN WITH MEDICAL HANDICAPS (formerly BCMH) (updated 2016) <br />CMH is a state and federally funded health care program of the Ohio Department of Health. Serving as a <br />safety -net insurance program and payer of last resort, CMH links children with special health care needs, and <br />their families, to services that can diagnose a possible medical condition (Diagnostic Program). It also assists in <br />obtaining payment for ongoing services their child might need (Treatment Program). A child must have a <br />qualifying medical condition to be on the diagnostic and treatment programs and must see a CMH approved <br />provider annually. The Treatment program also has income requirements. A child may be eligible from birth — <br />21 years of age and the Treatment program must be renewed annually. <br />REPRODUCTIVE HEALTH & WELLNESS/ FAMILY PLANNING <br />This program is designed to address the gap in reproductive health and wellness services. The program provides <br />direct care, education, outreach activities, HIV testing, pregnancy testing/planning, STD testing and treatment, <br />and referral to social services as necessary. <br />BREAST AND CERVICAL CANCER PREVENTION PROGRAM (BCCP) Northeast Region <br />This program forms partnerships throughout the Northeast Ohio area with the goal of reducing the mortality of <br />breast and cervical cancer. The program includes screening, case management, patient navigation, tracking, and <br />follow-up as well as public education. <br />
