Healthy Children Healthy Weights (HCHW) <br />HCHW is a childhood obesity prevention program that utilizes a policy, systems and environmental change <br />approach to promote a healthy weight and growth in all children from birth to five years old. The HCHW <br />program offers 15 hours of training and at least 3 technical assistance visits for early learning centers and family <br />child care homes on adopting best practices in building healthy habits, center menus, and policies. Early <br />learning programs that complete the HCHW program are eligible to become designated as an Ohio Healthy <br />Program (OHP). OHP is a statewide designation awarded to early learning programs that have committed to <br />providing the children they serve a healthier place to learn and grow. <br />HEALTH IMPROVEMENT PARTNERSHIP-CUYAHOGA (HIP-CUYAHOGA) <br />More than 100 community partners have come together as the HIP -Cuyahoga Consortium to build opportunities <br />for everyone in Cuyahoga County to have a fair chance to be healthy. The Cuyahoga County Board of Health <br />serves as the backbone organizations for the partnership, providing strategic guidance, managing day-to-day <br />operations, and guiding communications, community engagement and evaluation/measurement activities. HIP - <br />Cuyahoga selected to address four of the most pressing issues impacting the health of our county's residents <br />including: 1) Eliminating Structural Racism; 2) Healthy Eating and Active Living; 3) Linking Clinical Care and <br />Public Health; and 4) Chronic Disease Self -Management. The partnership intends to reassess the community's <br />needs every 3-5 years as needs evolve. <br />INDOOR AIR QUALITY <br />The Indoor Air Quality (IAQ) Program involves the investigation and testing of residences, schools and other <br />public buildings for various indoor air quality indicators on a complaint basis. <br />CREATING HEALTHY COMMUNITIES <br />Creating Healthy Communities (CHC) is committed to preventing and reducing chronic disease. Through cross - <br />sector collaboration, the program activates communities to improve access to and affordability of healthy food, <br />increase opportunities for physical activity, and assure tobacco -free living where Ohioans live, work and play. <br />By implementing sustainable evidence -based strategies, CHC is creating a culture of health. The three main <br />communities of focus for this program are currently East Cleveland, Euclid and Lakewood. <br />PERSONAL RESPONSIBILITY EDUCATION PROGRAM (PREP) <br />The goal of the PREP program is to reduce teen pregnancy and sexually transmitted infection rates for youth in <br />Cuyahoga County and four surrounding counties who are currently in foster care or the juvenile justice systems <br />by educating the direct -care staff in those systems to become trainers in evidence -based pregnancy prevention <br />programming. This train -the -trainer model seeks to enhance professional development at the local level for <br />direct care staff while providing sustainability for pregnancy prevention and life skills education for youth in the <br />foster care and juvenile justice systems. <br />RACIAL AND ETHNIC APPROACHES TO COMMUNITY HEALTH (REACH) <br />The REACH program seeks to implement policy, systems and environmental strategies that address two of the <br />four key priorities identified by HIP -Cuyahoga countywide health improvement plan — Healthy Eating and <br />Active Living (HEAL), and Chronic Disease Self -Management (CDM). Strategies are being implemented <br />across six Cleveland neighborhoods and one large section of East Cleveland, aiming to reach over 40,000, <br />primarily African-American, residents. The project's strategies target three chronic disease risk factors (poor <br />nutrition, physical inactivity, and poor linkages to quality health services). The strategies employed will lead to <br />increased communication and collaboration among community-based efforts; increased access to physical <br />activity opportunities; increased access to nutritious food within the target neighborhoods; and improved <br />cultural and linguistic competency within health care professionals serving the targeted population leading to <br />better blood pressure control among adults in the target neighborhoods. <br />
