RYAN WHITE <br />The Ryan White HIV/AIDS Part A program provides HIV-related services for those who do not have sufficient <br />health care coverage or financial resources for coping with HIV disease. The Cuyahoga County Board of <br />Health (CCBH) serves as the Program Administrator of the Cleveland Transitional Grant Area (TGA) which <br />serves the following Ohio Counties: Cuyahoga, Ashtabula, Geauga, Lake, Lorain, and Medina. The Ryan white <br />Part A program contracts with local community partners to provide services in the areas of: outpatient <br />ambulatory health care, medical case management, oral health, medical nutrition therapy, mental health, early <br />intervention, home health care, home and community-based health care, substance abuse inpatient and <br />outpatient, non-medical case management, medical transportation, outreach, food bank/home delivered meals <br />and psychosocial support services. <br />Community Assessments <br />CHRONIC DISEASE SURVEILLANCE <br />A specialized team of experts including data analysts, environmental health sanitarians, and public health nurses <br />address chronic disease clusters of major public health concern. The program consists of: 1) disease <br />identification and investigation of community risk factors; 2) appropriate environmental follow-up/sampling; 3) <br />education; 4) and community collaboration. <br />REPORTABLE DISEASE SURVEILLANCE/PUBLIC HEALTH PREPAREDNESS <br />This program encompasses reportable disease tracking and disease surveillance activities. The purpose of this <br />program is to increase public health response and capacity regarding infectious disease case management and <br />surveillance, acts of bioterrorism and other major public health events. Activities also include developing <br />emergency plans and supporting the County Emergency Operations Plan by serving as the lead agency under <br />Emergency Support Function (ESF) #8. This program is supported by the Public Health Emergency <br />Preparedness grant (PHEP) and the Cities Readiness Initiative (CRI) grant. <br />IMMUNIZATION ACTION PLAN (IAP) <br />This program is designed to raise childhood immunization rates in Ohio so that 90% of children will be up to <br />date by 24 months of age and 80% of adolescents are up-to-date. This is accomplished by implementing best <br />practices identified to increase immunization rates including immunization assessment, education and tracking <br />objectives. These efforts are delivered through education sessions and immunization rate audits provided to <br />Cuyahoga county pediatric and family practice providers. The program also includes outreach to guardians of <br />young children to promote the importance of vaccines to protect the health of young children. <br />MATERNAL AND CHILD HEALTH PROGRAM <br />This program is designed as an organized community effort to improve the health status of women and children <br />in Cuyahoga County, including assessment and monitoring of maternal and child health indicators at the <br />individual community or neighborhood level. The program also provides funding to partner agencies to address <br />child and adolescent health needs, as well as the needs of pregnant women and teens at risk for poor birth <br />outcomes. <br />CHILD FATALITY REVIEW <br />This program reviews circumstances surrounding the deaths of children in efforts to make recommendations for <br />actions to reduce preventable deaths in children in Cuyahoga County. <br />
