Services provided include: menu/diet analysis; jail menu <br />analysis and development; formula assistance; group and individual <br />education/training. <br />Program.lnquiries.and Respanses.(in hours)*.......................2 <br />FAMILY PLANNING SERVICES <br />The Family Planning Program provides preventive health care services such as <br />education and counseling; screening for sexually transmitted infections (STls) and <br />HIV; pregnancy diagnosis and counseling; contraceptive methods and/or <br />prescriptions for contraceptive supplies, when applicable; other medications; and <br />basic infertility services. Services are available to adolescents, women and men of <br />reproductive age and offered at the CCBH Parma clinic. <br />Program.lnquiries.and Reslaanses.(in hours)*......................43 <br />HIV/STD.Counseling..........................................2 <br />RYAN WHITE PART A GRANT <br />The Ryan White HIV/AIDS Part A program provides HIV-related services for those <br />who do not have sufficient health care coverage or financial resources for coping with <br />HIV disease. The Cuyahoga County Board of Health (CCBH) serves as the Program <br />Administrator of the Cleveland Transitional Grant Area (TGA) which serves the <br />following Ohio Counties: Cuyahoga, Ashtabula, Geauga, Lake, Lorain, and Medina. <br />The Ryan white Part A program contracts with local community partners to provide <br />services in the areas of: outpatient ambulatory health care, medical case <br />management, oral health, medical nutrition therapy, mental health, early intervention, <br />home health care, home and community-based health care, substance abuse <br />inpatient and outpatient, non-medical case management, medical transportation, <br />outreach, food bank/home delivered meals and psychosocial support services. <br />Program.lnquiries.and Respaanses.(in hours)*.....................134 <br />Community Assessments <br />
