CCBH 2016 SEMI-ANNUAL REPORT City of North Olmsted Page 5 <br />REPORTABLE DISEASE SURVEILLANCE/PUBLIC HEALTH PREPAREDNESS <br />This program encompasses reportable disease tracking and disease surveillance <br />activities. The purpose of this program is to increase public health response and <br />capacity regarding infectious disease case management and surveillance, acts of <br />bioterrorism and other major public health events. Activities also include developing <br />emergency plans and supporting the County Emergency Operations Plan by serving <br />as the lead agency under Emergency Support Function (ESF) #8. This program is <br />supported by the Public Health Emergency Preparedness grant (PNEP) and the <br />Cities Readiness Initiative (CRI) grant. <br />Program.lnquiries.and Respanses.(in hours)*.....................240 <br />Illness. Investigations/Disease Surveillance ........................45 <br />IMMUNIZATION ACTION PLAN (IAP) <br />This program is designed to raise childhood immunization rates in Ohio so that 90% <br />of children will be up to date by 24 months of age and 80% of adolescents are up-to- <br />date. This is accomplished by implementing best practices identified to increase <br />immunization rates including immunization assessment, education and tracking <br />objectives. These efforts are delivered through education sessions and immunization <br />rate audits provided to Cuyahoga county pediatric and family practice providers. The <br />program also includes outreach to guardians of young children to promote the <br />importance of vaccines to protect the health of young children. <br />Program.lnquiries.and Respanses.(in hours)*......................68 <br />Case. Management...........................................8 <br />MATERNAL AND CHILD HEALTH PROGRAM <br />This program is designed as an organized community effort to improve the health <br />status of women and children in Cuyahoga County, including assessment and <br />monitoring of maternal and child health indicators at the individual community or <br />neighborhood level. The program also provides funding to partner agencies to <br />address child and adolescent health needs, as well as the needs of pregnant women <br />and teens at risk for poor birth outcomes. <br />Program.lnquiries.and Respanses.(in hours)*......................7.2 <br />
