12.04 Employees shall participate in the AFSCME Dental Benefit Plan and Union Eye Care <br />Vision Plan. <br />12.05 Expected cost or premium is defined as the cost ol'all medical, hospital, prescription <br />drug, and related fees including, but not limited to administrative fees. <br />12.06 The Employer reserves the right to establish all plans, enrollment periods and regulations <br />12.07 The Employer will secure a commercial insurance policy whereby any bargaining unit <br />member killed in the line of duty shall receive to his spouse or estate, a payment of fifty <br />thousand ($50,000.00) dollars. Such payment will be made in accordance with the terms of the <br />commercial plan. <br />12.08 The Employer shall cause the Health Care Plan to be a qualified Plan under Section 125 <br />of the Internal Revenue Code. <br />ARTICLE; XIII CLOTHING ALLOWANCE <br />13.01 All employees shall maintain duty uniforms as mandated by the Rules and Regulations <br />13.02 All new police officers, immediately upon completing the selection process and prior to <br />appointment shall be provided two (2) duty uniforms, one (I) uniform cap, one (1) lightweight <br />Jacket, one (I ) winter_jacket, and one (1) pair of boots. All clothing provided and/or clothing <br />allowance paid shall be surrendered to the Employer in the event the employee fails to complete <br />the probationary period, including all issued equipment given by the Employer. <br />13.03 All full-time bargaining unit members shall receive an annual clothing allowance of one <br />thousand three hundred fifty ($1,350.00) dollars payable with the first regular pay check in <br />January for the duration of this Agreement. <br />13.04 Clothing allowance shall be pro -rated per month in the employee's final year of <br />employment. <br />13.05 In the event of damage to uniform items, including watches and prescription eyeglasses, <br />which occurs in the active discharge of an employee's duties enforcing the law. Duties such as <br />routine issuance of a citation or a routine accident investigation shall not be considered a cause <br />for replacement of damaged items.'T'he Employer shall pay the difference between the amount of <br />reimbursement from the insurance or court ordered recovery and the cost of repair or <br />replacement at current fair market value. I lowever, the Employer's monetary reimbursement <br />obligation for repair or replacement ol'the following articles shall not exceed the maximum <br />amounts set forth below: <br />Item Maximum Employer Cost <br />Watch $30.00 <br />Prescription Eyeglasses $100.00 <br />Uniform Jacket $200.00 <br />io <br />
