10.06 If an employee is terminated (voluntarily or involuntarily) prior to taking his vacation and <br />has completed one (I) year of employment with the Employer, he shal l receive the prorated <br />portion of any accrued but unused vacation at the time of separation. In case of death of an <br />employee, the unused vacation leave shall be paid to the estate. <br />10.07 A) An employee may have the option of using a personal day as a day of extended <br />vacation, with prior approval of' the Chief of Police or his designee. <br />B) The 1mployer may have the option of requiring the employee to take one week of <br />his vacation in increments of not less than rive (5) consecutive days. The remaining vacation <br />time can be taken as needed by the employee, for example, one or two days at a time. An <br />employee will give the supervisor at least forty-eight (48) hours advance notice in writing, In urn <br />emergency situation, verbal communication with the supervisor is appropriate and may justify <br />waiving advance notification. <br />ARTICLE XI PROBATIONARY PERIOD <br />11.01 The probationary period for all new employees shall continue for twelve (12) months <br />after the employee(s) successfully completes the Employer's Police Department's Field Training <br />Program. During this probationary period, the employee shall have no rights to appeal any form <br />of disciplinary action, including termination through the Grievance Procedure or to any Civil <br />Service Commission. <br />11.02 Should an employee voluntarily resign during their probationary period, and the City of <br />North Olmsted paid for their OPTA training, the employee is responsible to reimburse the city <br />the full training cost. The Employer may deduct said cost from the employee's final settlement <br />check. <br />ARTICLE XII HEALTH INSURANCE; <br />12.01 Commencing on ,January I, 2019, the Employer shall provide, pursuant to Appendix A, <br />single, family, employee and spouse, employee and child/children, medical insurance coverage <br />at the employee's option for each full-time employee. <br />12.02 Ernployee(s) participating in the city's medical insurance coverage program shall <br />contribute an amount equal to 13% percent of the plan premium or expected costs of such <br />medical plan, and the Employer shall contribute eighty-seven (87%) percent of such costs. The <br />employee contribution shall be withheld via payroll deduction not later than the first pay period <br />each month. <br />12.03 Commencing on January 1, 2019, the Employer will offer an I -IMO, PPO and I -IDI -IP - <br />HSA plans that provides for annual deductibles, co -pays and co-insurance as set forth in the <br />attached Plan designs, providing said plans contain deductibles, co -pays, co-insurance benefits <br />that are no less than those contained in Appendix A. <br />
