Exhibit A <br />completed in a manner consistent with the ODOT Safety Study Course manual and program <br />requirements. <br />The items listed below are in no particular order, and may ultimately be included in the final scope of <br />services for the subject project. These are provided solely for consideration in developing a <br />response submittal to the RFQ. The final scopes of work will be developed after notice of selection is <br />provided to the most qualified firm, with input from the City of North Olmsted. <br />• Turning movement traffic counts at all intersections in the study area <br />• Field review of the corridor <br />• Design Year (20 -Year) traffic analysis <br />• Signal warrant evaluation <br />• Auxiliary turn lane analysis <br />• Safety analysis of the corridor <br />• Draft and final study including corridor recommendations <br />• Safety funding application preparation <br />• Public meetings <br />• Potential design plans (Additional Services) <br />Requirements for Statements of Qualifications <br />General Instructions <br />Provide the information requested in the SOQ Content and Criteria in any order along with a letter <br />signed by an officer of the firm. Do not send additional forms, resumes, brochures or other material. <br />SOQs shall be limited to ten (12) single sided pages plus two (2) pages for the Project Approach. <br />Please adhere to these format guidelines in preparing SOQs: <br />• Use a minimum font size of 11 -point and maintain margins of 1" on all four sides. <br />• Use 8'/" x 11" paper only. <br />• Bind SOQs by stapling at the upper left hand corner only. Do not utilize any other binding <br />system. <br />• Do not provide tabbed inserts or other features that may interfere with machine copying. <br />SOQ Content and Criteria <br />1. List the types of services your firm provides and the number of years in business. Indicate the <br />following required pre -qualifications and experience, through a combination of prime consultant <br />and sub consultants: <br />Complex Roadway Design, Safety Study, Basic Traffic Signal Design, Traffic Signal Systems <br />Design, Limited Highway Lighting Design, Environmental Document Preparation — CE, <br />Environmental Document Preparation — EA/EIS; experience working with ODOT District 12 <br />Traffic, Planning and Safety staff and experience with Safety Studies and Safety Funding <br />2. List the project manager and other key staff members involved in the project and their relevant <br />experience and qualifications. List any sub consultants you propose to use for this project. <br />3. Describe the capacity of the staff and their ability to perform the work in a timely manner relative <br />to your current work load and availability. Provide a preliminary project schedule assuming a <br />start date of June 1, 2020. <br />
