(d) Exposure to contagious disease where quarantined by the Board of Health or <br />communicable to other employees. <br />(e) Pregnancy of the employee, birth of a child, and placement of a child with the <br />employee for adoption or foster care. <br />11.04 Paid Sick Leave: Full and part-time employees shall be granted sick leave as follows: <br />(a) Paid sick leave shall be earned and accumulated at the rate of four and six -tenths <br />(4.6) hours for every eighty (80) hours paid, excluding overtime. <br />(b) Pay for sick leave shall be at the einployee's straight time hourly rate (or portion <br />thereof if absent for less than a full day). <br />(c) If an employee is absent on sick leave for more than three (3) consecutive work <br />days, the Employer shall require the employee to present a physician's report <br />attesting to the employee's incapacity and that the employee was under the <br />doctor's care and is able to return to work. The F_,mployer will review the <br />attendance records of employees periodically, and at least once in each three (3) <br />month period. If it believes that an employee is developing an attendance <br />problem, it will notify the employee in xvriting that medical evidence will be <br />required for absences of three ( 3) days oi- less. When an employee's record <br />becomes satisfactory, the requirement for such medical evidence for short term <br />absences will be removed, and the employee will be notified in writing. <br />(d) Employees may use up to fifteen (15) work days per year of earned and <br />accumulated sick leave if necessitated by illness in the employee's immediate <br />tanilly. For purposes ofthis subsection. `"in7mediate fanlily" shall be interpreted to <br />be sponse, children, parents ofthe employee. regardless ofthe place of'residence; <br />and other relatives living within the household ofthe employee. <br />(e) When an employee with five (5) or more years of service retires, having received <br />PERS pension, dies or takes disability retirement and has accumulated sick leave <br />time, the retiring employee may convert one-half( 1 1/2) of the accumulated sick <br />leave time (with the City of North Olmsted) to pay on the basis of his/her rate of <br />pay at the time of retirement, but the maximum amount paid shall not exceed one <br />thousand (1000) hours. Employees hired after July 1, 1998, and have accumulated <br />sick leave with another political subdivision will transfer said sick leave and shall <br />use the transferred sick leave prior to sick leave accumulated while employed <br />with the City of North Olmsted. Sick leave carry, over from another political <br />subdivision shall not be used to calculate an employee's buy-out pursuant to the <br />above. <br />(t) Sick leave taken the day prior to or the day after a scheduled vacation or holiday, <br />will require a medical excuse prior to approval for payment. <br />11.05 General Leave Provisions: <br />(a) All leaves of absences when known in advance (and any extensions thereof) must <br />be applied for in advance and granted in writing on forms provided by the <br />Employer (copy to the employee).
