EXHIBIT A North Olmsted COVID-19 Small Business Relief Grant Eligibility Requirements The Business Must • Be physically located in the City of North Olmsted, Ohio • Be in good standing with the City of North Olmsted, State of Ohio and any other taxing authority (i.e. no tax delinquencies) • Have been established prior to April 1st, 2019 • Employ fifty or fewer payroll employees (1099/contract employees are not eligible) 1. Businesses with three (3) to fifty (50) employees are eligible for a maximum grant of $2,500. 2. Businesses with one (1) or two (2) employees will be eligible for a maximum grant of $1,000. 3. For purposes of this criteria, "employees" shall mean individuals who regularly work at least 20 hours per week and who are individuals who are employed as employees and provided an IRS Form W-2 at the end of each calendar year. For purposes of determining whether the business employs the requisite number of individuals, the principal owners of the business may be included to meet the minimum of individuals employed. • Rent or own a retail or commercial space (home based businesses are not eligible) • The businesses shall NOT be one of the following, regardless of its general eligibility above: 1. Any financial institution that makes loans or issues credit to the public, including, but not limited to, banks, credit unions, payday lenders, or any other similar business 2. Any chain business with more than three (3) locations 3. Professional services business (law, of accounting firm, or realty/ real estate company of any type) 4. Medical, dental, or veterinary businesses 5. Schools or daycares 6. Any business that continued full operation through 2020 as an "essential business" • Business must show how Covid-19 has interrupted, reduced, or impaired operation 7. E -Commerce 8. Alcohol, Liquor and Wine stores 9. Vaping and tobacco businesses 10. Gas stations 2
