EXHIBIT A <br />North Olmsted COVID-19 Small Business Relief Grant <br />Eligibility Requirements <br />The Business Must <br />• Be physically located in the City of North Olmsted, Ohio <br />• Be in good standing with the City of North Olmsted, State of Ohio and any other taxing authority <br />(i.e. no tax delinquencies) <br />• Have been established prior to April 1st, 2019 <br />• Employ fifty or fewer payroll employees (1099/contract employees are not eligible) <br />1. Businesses with three (3) to fifty (50) employees are eligible for a maximum grant of $2,500. <br />2. Businesses with one (1) or two (2) employees will be eligible for a maximum grant of $1,000. <br />3. For purposes of this criteria, "employees" shall mean individuals who regularly work at least <br />20 hours per week and who are individuals who are employed as employees and provided an <br />IRS Form W-2 at the end of each calendar year. For purposes of determining whether the <br />business employs the requisite number of individuals, the principal owners of the business <br />may be included to meet the minimum of individuals employed. <br />• Rent or own a retail or commercial space (home based businesses are not eligible) <br />• The businesses shall NOT be one of the following, regardless of its general eligibility above: <br />1. Any financial institution that makes loans or issues credit to the public, including, but not <br />limited to, banks, credit unions, payday lenders, or any other similar business <br />2. Any chain business with more than three (3) locations <br />3. Professional services business (law, of accounting firm, or realty/ real estate company of any <br />type) <br />4. Medical, dental, or veterinary businesses <br />5. Schools or daycares <br />6. Any business that continued full operation through 2020 as an "essential business" <br />• Business must show how Covid-19 has interrupted, reduced, or impaired operation <br />7. E -Commerce <br />8. Alcohol, Liquor and Wine stores <br />9. Vaping and tobacco businesses <br />10. Gas stations <br />2 <br />
