c) If the applicant does not qualify based upon section (b), a written examination as to the iqualifications and competency to perform the wOrk for which the application is sought shall be con- ducted by the Building Commissioner based on the minimum standards as set forth in the applicable codes and ordinances. 1321.03 TIME AND FEE a) The license shall be vailid for the calendar year in which and shall be renewed annually thereafter. A fee of Twenty-Five Dollars ($25.00) shall be paid at the time of the application and at each renewal thereafter. 1321.04 INSURANCE a) Each applicant for a license shall furnish evidence of insurance for bodily injury in the amount of One Hundred Thousand Dollars/Three Hundred Thousand Dollars ($I00,000/$300,000)~ and for property damages in the amount of at least Fifty Thousand Dollars ($50,000). 1321.O5 BOND a) When evidence discloses that the licensee has refused, failed or neglected to correct or abatelviolations of any applicable ~ode or ordinance in performance of any work done pu'rsuant to a license within a reasonable time after having been notified by the Building Commissioner, the Building Commissioner [nay require the licensee to furnish a per- formance bond in the sum of Five!Thousand Dollars ($5,000) guaranteeing full and faithful compliance by [he licensee with all provisions of any applicable Code or Ordinance of [he City of Lakewood and binding the surety thereon to correct or abate any violations of any applicable Code or Ordinance of the City of iLakewood whenever the applicant for a license named as the principal ion such bond refuses, neglects or fails to correct or abate such vi!olation within a reasonable time limit set by the Building CommisSioner. 1321.O6 LICENSING BY THE BUILDING COMMISSIONER a) Upon receipt of a proper!ly completed license application and upon compliance with Sections 132i1.02, ]321.03, 1321.04 and 1321.05, the Building Commissioner shall issue a license to the applicant which shall be valid only for the!current calendar year or disapprove the application for one of the several reasons as set forth in 1321.07. 1321.O7 REVOCATION' OR DISAPPROVAL OF LICENSE a) The Building Commissione:r may disapprove or revoke any license or renewal thereof issued under the provisions of this Chapter for one or several of the following reasons: (1) Misrepresentation of a material 'fact by the applicant in obtaining the license or renewal thereof. (2) The use of the license by the licensee in obtaining a Building Permit for work to be performed by a contractor or builder other than the licensee. (3) Violation of or non-compliance with any provision of any applicab]e Code or Ordinance of the City of Lakewood in the performance of any work done~pursuant to a ][cense issued here- under.
