ATTACHMENT D <br /> <br />Agreement Number 3440 <br /> <br />QUALITY CUSTOMER SERVICE SUPPORT <br /> <br />XEI~OX BUSINESS MANAGEMENT SYSTEM SOFTWARE LICENSE <br /> <br />This Attachment entitles the Customer to use the Quality Customer Service telephone <br />facility for assistance with problems concerning the Customer's licensed business <br />applications. <br /> <br /> Charges <br /> <br /> t4.1 Upon execution of this Agreement and each year thereafter, Customer shall <br />pay XerOx the following charges. Charges may be prorated for a partial initial year. <br /> <br /> ~.2 Customer is entitled to 300 calls per year for an annual charge of $6,000 per <br /> <br />user. <br /> <br />at the rate of $35 per 'call. <br /> <br />~.3 Catls in excess of 300 per year will be billed at the rate of $20 per call. <br /> <br />g.~ Calls received from Customers who have not subscribed to this service will be <br /> <br />Terms <br /> <br /> 5.1 Duration. This Agreement shall take effect on <br />and remain in eHect until February 1, 19gq. <br /> <br /> At that time, this Agreement shall automatically renew.and continue to renexv, for <br />one (1) year periods, unless terminated by either party upon thirty (30) days written notice <br />to the other party prior to renewal. <br /> <br /> 5.2 Billing and Credit Termsr Thirty (30) days prior to the effective date (5. I <br />Duration) of this Attachment, Customer shall pay Xerox $ , <br />which is the prorated annual charge for Quality Customer Service Support to the next <br />annual billing date. Each renewal date thereafter, Gustomer shall be billed the annual <br />charge then in effect. <br /> <br /> 5.3 Cancellation. This Attachment is cancellable after the initial year by either <br />party upon thirty (30) days written notice to the other party. <br /> <br />No on-site consulting is provided in this Attachment.
