II <br /> <br />III. <br /> <br />IV. <br /> <br />.V/ne Department agrees to notify the designated Agency within <br />twenty-four (24) hours of receipt of information identifying <br />the Agency as meeting one of the two'conditions specified in <br />Item I above. (By telephone to be followed up in %~iting). <br /> <br />The Department, active in cases where it is unaware of the <br />Agency's involvement with the client, upon becoming aware of <br />the Agency-client involvement and before disposition of its <br />report, shall advise by phone the Agency within three (3) days <br />of its findings and any recommendations for on-going activities <br />and services. <br /> <br /> The Agency agrees to perform the following duties: <br /> <br />A. Initiate an assessment of non-emergency reports of abuse, <br /> neglect or exploitation which are assigned by the Depart- <br /> ment within three .(3) working days of the date of the <br /> assignment. <br /> <br />Provide, at the time of the initial interview, a <br />written notice of intent to assess to the client <br />(with a copy to the Department) in language that <br />is reasonably understandable by the subject of <br />the investigation. <br /> <br />Upon completion of the assessment the Agency shall <br />determine whether the client is in need of on-going <br />protective services and report this together with the <br />Agency's'complete findings to the Department within <br />thirty (30) days of assignment. This report will in- <br />clude at a minimum: <br /> <br />1. Date of assignment. <br />2. Nature of the complaint. <br /> <br />3. Names of people contacted. <br />4. Dates of contacts. <br />5.
