VI. <br /> <br />The Department and the Agency agree to designate liaisons <br />whose roles will be to: <br /> <br />A. Refer reports of abuse, neglect of exploitation; <br /> determine if the complaint should be assigned to <br /> the designated Agency and/or Department; ~ <br /> <br />B. Receive reports of abuse, neglect or exploitation <br /> from the Department and/or Agency; <br /> <br />C. Report findings to the Department and/or 'Agency. <br /> <br />D. IDsure that reports are received in a timely manner <br /> and incorporated into the Department's mandatory <br /> State reports; <br /> <br />Develop and implement any needed additional pro- <br />cedures; <br /> <br />F. Monitor the effectiveness of~the Agreement. <br /> <br />In the event that the intervention of Probate Court is <br />required the following steps will be taken: <br /> <br />AGENCY RESPONSIBILITY <br /> <br />DEPARTMENT RESPONSIBILITY <br /> <br />Submit a written report <br />to the Department which <br />identifies the circum- <br />stances necessitating <br />the filing. <br /> <br />Give testimony at Probate <br />as required. <br /> <br />Provide all documents <br />to be used as evidence. <br /> <br />Review report submitted <br />by the Agency and give <br />approval for the filing. <br /> <br />2.
