LETTER OF AGREEMENT <br /> between <br /> <br />Lakewood Senior Citizens, Inc. and City of Lakewood <br /> <br />RE: <br /> <br />Westerly II - Service Coordinator <br />14306 Detroit Avenue <br /> <br />The City of Lakewood agrees to assign a member of its full time staff to fill the <br />position of Service Coordinator, as defined by the Department of HUD, at the <br />above referred senior building operated by Lakewood Senior Citizens, Inc. The <br />contract term is three years, 2001 through 2003. <br /> <br />The staff member assigned is to perform social service duties during hours and <br />at locations as agreed upon by Lakewood Senior Citizens, Inc. and the <br />Lakewood Department of Human Services, Division of Aging. Provision of a <br />Service Coordinator is contingent upon continued approval for funding through <br />the Department of HUD and adequate appropriation by the City of Lakewood. <br /> <br />The City of Lakewpod full time staff member shall be a Licensed Social Worker <br />and have experience working with the elderly. The staff person assigned must <br />be approved by Lakewood Senior Citizens, Inc.; such approval shall not be <br />unreasonably withheld. <br /> <br />Costs to the City of Lakewood to be reimbursed by Lakewood Senior Citizens, <br />Inc.: <br /> <br />Salary Fringe Benefits Grant Award City Match <br /> <br />Year 1 $33,600 $5,712 $39,312 <br />Year 2 $35,280 $5,998 $41,278 $9,256.50 <br />Year 3 $37,044 $6,298 $43,342 $9,718.50 <br /> <br />The annual salary increase is based on 5%, which is the maximum that HUD <br />allows. The fringe is 17% of the salary figure. <br /> <br />Estimate for the .salary and fringe benefits for the term of the contract is <br />$123,932. In addition, a mileage and training and development allowance of <br />approximately $1,000 per year shall be included. <br /> <br />Suitable office space and equipment to be provided by Lakewood Senior <br />Citizens, Inc. subject to approval by the Lakewood Department of Human <br />Services, Division of Aging. <br /> <br /> <br />
