Task I Phase I Environmental Site Assessment <br /> <br />HAI will atr~.~pt to identify all known or suspected releases of tmzardous substances or petroleum, which <br />have or may have occurred on, underlying or emanating from the Property. The assessment will also <br />include-a limited asbestos survey. The Phase I will be conducted in accordance with the requirements of <br />Ohio Administrative Code (OAC) 3745-300-06. <br /> <br />Schedule: Complete - (Full Report in Application Attachment) <br />Cost: $4,480 - (Cost not included in Loan Request Application) <br /> <br />Task 2 UST Eligibility Investigation <br />HAI will conduct an investigation of the suspect 500-gallon gasoline tank. HAI will conduct a soil <br />sampling program, including collecting soil and groundwater samples in and around the suspected <br />location of the former gasoline UST. <br /> <br />Schedule: Complete- .(FuI1 Report in Application Attachment) <br />Cost: $4,763 - (Cost not.included in Loan Request Application) <br /> <br />Task 3 Groundwater Monitoring Well Installafion/Developmeot/Snmplino <br />To evaluate the two nearby off-site underground storage tank~ releases, a monitoring well installation <br />program will be implemented and will consist of installing three monitoring Wells at the Property. HAI <br />will monitor each well for BTEX/MTBE and n-hexane in accordance with EPA Method 8021. Each well <br />will be resampled to evaluate the groundwater protection provision pursuant to OAC 3745-300-07 and <br />-10. The City of Lakewood shall then file the appropriate'Deed Restrictions on the Property concerning <br />well-water usage, thereby eliminating the possible installation of groundwater wells for potable use. <br />Scl~edule: November 2001 <br />Cost: $20,000 <br /> <br />Task 4 Asbestos Removal and Disposal <br /> <br />A certified asbestos abatement contractor will be retained by the City of I. akewo.od to remove and <br />properly dispose offsite all asbestos containing materials identified in Buildings I and2. As such, HAI <br />will prepare Asbestos Removal Specifications and provide project oversight. <br /> <br />Schedule: December 2001 and January 2002 <br />Cost: $175,000 <br /> <br /> <br />
