is hereby authoriZed to paY from OtY funds the actual premium rate in cash based on <br />the difference between the family plan rate and the single plan rate, but in no event <br />shall such cash payment exceed forty-two dollars and thirty-one cents ($42.31) per <br />month. Effective April 3, 1978, any new employee from that date forward shall not <br />receive any cash payment in lieu of hospitalization coverage. <br /> <br /> (d) Such payment by the City shall be distributed among City funds according <br />to the enrollment of each division participaUng in the City=s employee hospitalization <br />plan. Cost allocaUon shall be determinedbased on the number of employees in each <br />division participating in the hospitalization plan. The total costs of hospitalization <br />services shall be allocated according to the percent of enrollment each division <br />represents as compared to the total enrollment on the City--s hospitalization plans. <br />Where there is no provision in grant programs to pay for employee hospitalization costs, <br />the Director of Finance will allocate that cost to the funds based upon the total <br />allocation of all other hospitalization costs. <br /> <br /> (e) The hospitalizaUon coverage provided for in this section shall be for that <br />coverage commencing April 12, 1978. <br /> <br /> (0 The Director of Finance is hereby authorized to pay from City funds one- <br />half of the actual premium rate of dental insurance coverage as may be established by <br />the City for all annual salaried .e. mployees including Foremen Class A employees and the <br />Chiefs of the Divisions of Police and Fire but excluding all other members of the <br />Divisions of Police and Fire and full-time permanent hourly employees covered by a <br />collective bargaining agreement with the City. <br /> <br /> 149.151 HOSPI'i'ALIZAT/ON COVERAGE FOR COUNCILMEN, <br /> (a) Commencing January 1, 1982, the Director of Finance is hereby <br />authorized to pay from City funds the actual premium costs of hospitalization coverage, <br />and any subsequent increase in costs therefore, if any, for Councilmen elected at larcje <br />in the Oty who are enrolled in the City's hospitalization plan. <br /> (b) Commencing 3anuary 1, 1984, the Director is hereby authorized to pay <br />from City funds the actual premium costs ..of hospitalization coverage including any <br />increase in costs therefore, if any, for Councilmen representing each of the four wards <br />in the City who are enrolled in~ the City's hospita;!zatioh plan. <br /> <br /> 149.16 PAYI~IENT OF DUES TO.LABOR ORGANIZAI'ZONS. <br /> (a) · Upon reCeipt of written authorization from an employee of the City, the <br />Director of Finance is directed to check off on the earnings of the employee the amount <br />indicated on such authorizaUon for the payment of dues to the employee's labor <br />organization. The authorization shall be revocable by written notice upon the will of the <br />employee. <br /> <br /> (b) The Director of Finance shall transmit the sum so collected, once each <br />month, to the labor organizaUon indicated on the employee's authorization. <br /> <br /> 149.17 RESZDENCE REI~UIREMENTS FOR POLZCE AND FZRE <br />DI¥1SIONS. <br /> (a) An applicant for a position as a sworn member of the Division of Poi]ce or <br />the Division of Fire sha;l be disqualified for appointment unless he is a bona fide <br />resident of the area. outlined in the residence requirement map that follows the text of <br />this ~¢hapter, bu~ a Person who is not a resident of the area designated in such map <br />may be appointed, provided that within six months of appointment such person <br />becomes a resident. Failure to comply with this section shall result in automatic <br />termination and forfeiture of office or position. <br /> <br /> (b) Where, due to part-time requirements, unique skill requirements, location <br />of work or other reasons, imposition of the residenCy requirement set forth in <br />subsection (a) hereof is impractical or othe.rwise precludes securing'service necessarY <br />for the good of the City, positions may t>e exempted from such requirement by <br />ordinance of Council. <br /> <br /> (c) As used in this section, '!resident" means a person who maintains a <br />residence which is the actual bona fide domicile of the individual where he normally <br />eats, sleeps and maintains normal household and personal effects, and which <br />represents his domiciliarY address for purposes of taxation, license registraUon, voting <br />and otherwise. <br /> <br /> <br />
