4. Pinancial and Statistical Repor~_~. Grantee claims for reimbursement are <br /> due monthly at ~he Agency's office on or before ~he tenth (10~h) calendar day <br /> following ~he month in which the reimbursable costs are incurred. The Grantee may <br /> <br /> submit a final (13~h month) claim for reimbursement to ~he Agency before ~he end of <br /> ~he first month following ~he last month of ~his agreement period. No payment for <br /> <br /> services under this agreement will be made to reimburse Grantee costs which are <br /> submitted on claims for reimbursement that are received by the Agency <br /> after January 31, 199I (~hir~eenth month). Contract amounts which are not spent by <br /> the Grantee will not accrue or carry forward to a future grant period. The Grantee <br /> agrees to submit monthly program income remittances in the amount of Title III C <br /> program income collected for each month of this Agreement period (if this agreement <br /> covers a nutrition service) and program income reports (for all services under ~his <br /> agreement) to the Agency by the tenth (10th) calendar day following the month in <br /> which the said collections were made. The Gr~tee agrees-to submit accurate <br /> quarterly statistical reports to the Agency quantifying units and clients served <br /> under this agreement on the forms provided by the Agency. The Grantee agrees to <br /> <br /> submit these quarterly statistical reports to the' Agency on or before the tenth <br /> <br /> (IOth) calendar day of April,- July and October in 1989, and January 1991, for <br /> <br />service performed during the three calendar months preceding those dates. If said <br />reports are not received by the Grantee on or before the said due date, the Agency <br />will will, hold funds which the Grantee seeks as reimbursement until such reports are <br />received and approved by the Agency. If the Grantee is selected to be in the <br />treatment group of service providers for the 199~ Title III Quali~y Assurance <br />program, the Grantee agrees to establish and implement a plan approved by the <br />Agency that has as its purpose to comply with the standards of said program as <br />outlined by the ODA and the Scripps Gerontological Center. <br /> <br />Page 3 of I0 <br /> <br /> <br />
