$. Moni~orznq. The Agency may conduc~ on-si~e monitoring of a service(s) for <br /> which funds are being reimbursed under ~his Agreemen~ a~ any ~ime during ~he no~ai <br /> working hours of ~he Gran~ee wi~h no prior notification'necessary, and ~he Gran~ee <br /> agrees ~ha~ representatives of ~he Agency shall be given full access ~o the <br /> premises upon which such service{s) is being provided. <br /> <br /> 6. Record Accessibili ~y and Re~en~ion. The Gran~ee agrees ~o allow <br /> representatives of ~he Agency and ODA access ~o all programmatic, fiscal, and o~her <br /> records rela~ed ~o ~he service(s) for planning, auditing, and monitoring purposes <br /> a~ any ~ime during ~he no~-mal working hours of ~he Gran~ee wi~h no prior <br /> notification necessary. The Gran~ee agrees ~o keep clien~ information including <br /> bu~ no~ limited ~o: name, address, ~elephone number, age, minority s~acus and <br /> handicap s~atus. The Gran~ee agrees ~ha~ said records shall be re~ained and shall <br /> <br /> be. accessible ~o ~he Agency for a period of three years following ~he period of <br /> ~his Agreement. The Agency agrees ~ha~ all information con~aiBed, in said records <br /> <br /> will be ~rea~ed in accord wi~h all applicable legislation 9uaranceein9 privacy. <br /> <br /> 7. Inventory of Equipment.. The Gran~ee agrees ~ha~ all furniture and <br />equ~ipment purchased entirely or paz~ially with Fed~eral funds under ~his agreemen~ <br />shall be inventoried and properly identified as Federal proper~y. Should Title III <br />funding for Gran~ee cease, the Gran~ee agrees ~o submi~ a plan for Agency approval <br />~o use said furniture and equipmen~ for othher active services for older persons. <br />However, if no such use for said furniture and equipmen~ is identified by Gran~ee <br />or if Agency does not approve said plan, ~he Gran~ee agrees either ~o.. <br /> <br /> A. Sell the furniture and/or equipment a~ a fair marke~ value and repay the <br /> Agency the proceeds minus the prorated share of the originai cost of the <br /> furniture and/or equipmen~ purchased wi~h non-federal sources. <br /> <br /> B. Turn over ~he excess propez-ty ~o the Agency for distribution ~o other <br /> services ~o older persons. <br /> <br />Page 4 of 10 <br /> <br /> <br />
