15.05 Military Leave <br />Employee members shall be granted leaves of absence for military duty in accordance with state <br />and federal laws and local ordinances. <br />15.06 Leaves of absence without pay or other fringe benefits may be granted by the City at its <br />discretion. <br />15.07 Maternity Leave <br />Female employee members shall be granted maternity leave in accordance with the provisions' <br />applicable to any other illness or non -duty related injury. <br />ARTICLE 16 HEALTH INSURANCE <br />16.01 The City agrees to provide for full-time employees and their dependents a choice of health <br />care plans, provided the City may change either carriers or delivery systems if the benefits and <br />provider networks are comparable to the present plan. The City shall not offer less than two (2) <br />plans; selected by the City. Prior to changing health care delivery systems, the City will meet and <br />confer with the FOP. <br />16.02 Effective January 1, 2020, for employees electing coverage under the 90/10 Plan, monthly <br />employee premium contributions shall be determined by adding the SERB statewide average <br />increase as published in the SERB Health Insurance 2019 The Cost of Health Insurance in the <br />Public Sector to the 2019 rates. Any employee premium contribution increase, as described in this <br />section, shall be capped at ten percent (10%). <br />For the 90/10 Single Plan the 2020 monthly employee premium calculation will be 2019 single <br />plan monthly premium rate multiplied by the 2019 SERB average for single plans plus the 2019 <br />single plan monthly premium rate ($104.56 x 7.4% = $7.74 + $104.56 = $112.30). <br />For the 90/10 Family Plan the 2020 monthly employee premium calculation will be the 2019 <br />family plan monthly premium rate multiplied by the 2019 SERB average for family plans plus the <br />2019 family plan monthly premium rate ($191.70 x 5.9% = $11.31 + $191.70 = $203.01). <br />Effective January 1, 2021, for employees electing coverage under the 90/10 Plan monthly <br />employee premium contributions shall be determined by adding the SERB statewide average <br />increase as published in the SERB Health Insurance 2020 The Cost of Health Insurance in the <br />Public Sector to the 2020 rates noted above. <br />Effective January 1, 2022, for employees electing coverage under the 90/10 Plan monthly <br />employee premium contributions shall be determined by adding the SERB statewide average <br />increase as published in the SERB Health Insurance 2021 The Cost of Health Insurance in the <br />Public Sector to the 2021 rates noted above. <br />13 <br />
