The form of such statement sha.11 be prescribed by the Mayor. Failure to comply with the requirements of this section shall subject such license holder to the penalty prescribed in Section 15 hereof and each day's failure or neglect shall cons•17itute a separate offense. - Sec. 6. Fees for Public Vehicle Licenses. For the licensing of each pubic vehicle under Sec ion 2 heroof,_.a fee of en ollars 010.00) per annum shall be charged, provided that in the case of the issuance of arty such license on and after October first in any year, half of such fee shall be charged. Sec. 7. Suspension or Revocation of Public Vehicle Licenses. The liayor shall revoke or suspend any license granted under.section.,2 hereof for failure of the licensee to maintain surety bond or liabilit;~ insurance in accordance with the requirements of Section ~., or to maintain such public vehicle in clean, fit and thoroughly safe condition for the transportation of passengers, or in the event such public vehicle shall be used for immoral or illegal purposes. Any license revoked by reason of the use of the licensed vehicle for immoral or illegal purposes shall not be reissued or reinstated. Any license suspended or revoked for the other causes herein specified may be reinstated or reissued when the cause of said suspension or revocation shall have been cured and upon payment of the cost incw°red in such reinstatement. (a) He shall be of the age of twenty-one (21) years or over, shal]. be an American citizen or shall have declared his intention to become such citizen, and shall be a bona fide resident of Cuyahoga County for at least thirty (30) days next preceding the date of such application; (b) He shall be of sound physa.que with good eye-sight and not subject to epilepsy, vertigo, k~eart®trouble, or any other infirmity of body or mind whiciz might render him unfit for safe operation of a public vehicle. (c) He shall be able to speak, read and write the English language. (d) He shall be clean in dress and person, and not addicted to the use of intoxicating liquors or drugs. (e) He shall. submit the names and addresses of at least two persons not related to him as character references. (f) He shall submit a statement setting forth his full name, residence, places. of residence for the five years previous to moving to his present address, age, color, height, color of eyes and hair, place of birth, length of time he has resided in Cuyahoga County, whether a citizen of the United States, places of previous employment, whether married or single, whether he has ever been arrested or convicted of a felony or misdemeanor, whether he has been summoned to court, whether he has previously been licensed as a driver or chauffeur and, if
