Sec, 3, Zabelling. In manufacturing goods or articles where " blindr' <br />workers are employed, the product may only be labelled nade by r'blind" worlcers_ <br />if seventy-f ive percentum of the direct labor is "blind" labora. <br />Sec. 4. Permits. <br />(a) (1) No person, f irm or corporation or its distributor may <br />manu£acture, sell, market or otherwise dispose of any <br />goods or articles to the public which, by way of any <br />imprint, stamp, label, firm name or other printed matter <br />indicate that said goods or articles were made by "blind" <br />workers or which by word of mouth or otherwise have been. <br />represented to be made by "blind" workers without f irst <br />obtaining a permit in writing from the Chief of Police <br />of the Village of Alayfield upon a form prescribed by him. <br />(2) None of the provisions of this section are meant to <br />apply to any person, f irm or corporation marketing goods <br />or articles made by "Ulind" workers on the pretuises of <br />an establislzed retail grocery, department or variety <br />store permanently located within the Village of Mayiield. <br />(b) Issuance of permits as provided for in Section 4(a) shall be <br />upon application being submitted to the Chief of Police accompanied <br />by an aff idavit ( as outlined in Section 4(c) and a fee of $5.00 <br />for each person, firm or corporation, plus no more than One Dollar <br />? ($1.00) f or each agent or representative listed pursuant to 4(c) <br />(3) below. <br />(c) Every person, f irm or corporation or its Village ofMayf iel.d <br />distributor wishing to secure a permit shall submit the affidavit <br />? of said person, or if a firm or corporation, the affidavit of <br />its President setting £orth the following: <br />(1) Name, local address and phone number of the applicant. <br />(2) Name, local address and phone number of its President <br />or local representative. <br />(3) i2ethod of selling to be employed, that is, phone, house- <br />to-house solicitation, etc. The nanes of individual agents <br />or representatives including solicitors of all types, <br />whether telephone or house-to-house, and delivery men <br />which said applicant believes will represent said applicani;o <br />(4) Whether products to be sold are "blind" products. <br />(5) Assertion that said products meet all the standards <br />set forth herein.
