Bereavement Leave <br />In the event of c3eath in the employee's immediate family, he <br />shall be entitled to ,three (3) days off, or in the case of a member of <br />the Fire Department under the rank of Chief, one twenty-four (24) hour <br />shift. The "immediate family" shall include the spouse, chilclren, <br />parents, brothers, sisters, spouse's parents and grandparents. Should <br />an exception to the af orementioned apply, such exception to be <br />authorized by the Mayor. This leave shall not be charged to any other <br />leave provision provided by the Ordinance. <br />I. Hospitalization and Medical Coverage. <br />1. The municipality shall pay the premiums f or all permanent <br />full-time employees insured under a contract of hospitalization and <br />medical coverage selected by Mayfield Village. <br />2. Permanent full-time employees of the municipality not <br />presently insured and new permanent full-time employees of the <br />municipality may join the plan at such intervals as may be authorized <br />by the municipality's medical insurance carrier, subject to the <br />approval of the Mayor and Finance Director. <br />3. Any permanent full-time employee who is otherwise eligible to <br />be covered under a contract of hospitalization and medical coverage <br />but voluntarily withdraws from the above-referenced insurance coverage <br />shall be paid the amount of One Thousand Dollars ($1,000.00) for each <br />year the employ ee is employed by Mayfield Village but does not request <br />coverage. The payment authorized herein shall be made in equal <br />quarterly installments. <br />J. Overtime. <br />1. Overtime will be paid in accordance with the Fair Labor <br />Standards Act, as amended. <br />2. All overtime shall be approved in advance by the Mayor unless <br />there exists an emergency that threatens the health, saf ety and <br />welfare of the municipality. <br />K. Travel Allowance and Conf erence Expense. <br />1. Twenty cents ($.20) per mile is hereby allowed to reimburse <br />employees of the City who are required to make trips on City business <br />in their own automobiles, other than transportation to and from work, <br />to be paid from the Treasury upon approval of the Mayor and the <br />Director of Finance.
