ORDINANCE NOo 87-8 Page -11- 3o Upon retirement all full-time employees shall be entitled to receive credit ana payment for accumulated but unused sick leave not to exceed thirty (30) days provided that they have been employed by the Village for ten (10) years or more. For purpo.ses of this section, an employee shall be considered to have retired when such employee takes action that is inconsistent with any reasonable intent to continue employment with the municipality. 4. If a full-time employee is scheduled to work on a holiday enumerated in Section 6(a), and cannot work due to illness, that day shall be considered a holiday day off and shall not be counted or compensated as a sick day. H. Bereavement Leave In the event of death in the employee's immediate family, he shall be entitled to three (3) days off, or in the case of a member of the Fire Department under the rank of Chief, one twenty-four (24) hour shift. The "°immediate family" shall include the spouse, children, parents, brothers, sisters, spouse's parents and grandparents. Should an exception to the aforementioned apply, such exception to be authorized by the Mayor. This leave shall not be charged to any other leave provision provided by the Ordinance. I. Hospitalization and Medical Coverage. 1. The municipality shall pay the premiums for all permanent full-time,employees insured under a contract of hospitalization and medical coverage selected by Mayfield Village. 2. Permanent full-time employees of the municipality not presently insured and new permanent full-time employees of the municipality may join the plan at such intervals as may be authorized by the municipality's medical insurance carrier, subject to the approval of the Mayor and Finance Director. 3. Any permanent full-time employee who is otherwise eligible to be covered under a contract of hospitalization and medical coverage but voluntarily withdraws from the above-referenced insurance coverage shall be paid the amount of One Thousand Dollars ($1,000.00) for each year the employee is employed by Mayfield Village but does not request coverage. The payment authorized herein shall be made in equal quarterly installments. J. Overtime. le Overtime will be paid in accordance with the Fair Labor Standards Act, as amended. 2. All overtime shall be approved in advance by the Mayor unless there exists an emergency that threatens the health, safety and welfare of the municipality.
