' <br />ADDENDUM TO <br />AN AGREEMENT BETWEEN MAYFIELD VILLAGE, OHIO AND <br />THE FRATERNAL ORDER OF POLICE LODGE #57 <br />MAYFIELD VILLAGE DIVISION EFFECTIVE JANUARY 1, 2000 <br />THROUGH DECEMBER 31, 2002 <br />This AGREEMENT is made and entered into this Ag day of ., 2002. <br />WHEREAS, the Village has been forced to change carriers and the benefit levels <br />for employee medical insurance including prescription riders and life insurance and forced <br />to change carriers for dental insurance; <br />WHEREAS, pursuant to Section 28.1 of the Collective Bargaining Agreement <br />entered into between the Village and FOP Lodge #57, any decision by the Village to <br />change carriers for the benefit plan is to be discussed with the FOP before implementation <br />and in the event there is any reduction in the benefit level, this provision requires a mutual <br />agreement between the Village and the FOP before implementation; <br />WHEREAS, as a result of the need to change carriers and the benefit plan and in <br />accordance with Section 28.1 of the Collective Bargaining Agreement, a health care task <br />force was established to study the problem of health care and health care cost containment <br />and formulate recommendations to be considered by the Village; <br />WHEREAS, the Village considered such recommendations and obtained new <br />medical and dental group insurance resulting in a reduction in the benefit level; and <br />WHEREAS, an agreement has been reached between the Village and the FOP to <br />address the reduction in the benefit level. <br />THEREFORE the parties agree as follows: <br />That Article 28 of the Agreement entered into between the Village and the FOP <br />Lodge #57 effective January 1, 2000 through December 31, 2002 is hereby amended to <br />include the following: <br />1. During the term of this Agreement, the Village will maintain a level of benefits <br />provided by Anthem PPO and MetLife for medical and dental group insurance, including <br />prescriptions riders and life insurance and is acceptable to the FOP as replacement for a <br />previous medical and dental group insurance through Anthem traditional and Fortis <br />Benefits. (See Plan descriptions attached hereto and incorporated herein at Exhibit "A".) <br />? <br />?? ?
