Agreement Between Mayfield Village, Ohio and the International Association Firefighters, Loca12619 <br />the benefit plan come about as a result of the Village's inability to secure previous benefit levels <br />through it broker and or agent, such reductions or changes will not be subject to the expedited impasse <br />procedure mentioned herein, provided that such inability to secure a previous benefit level is stated by <br />the health care provider or its agent and such refusal is directly verifiable by the Union <br />It is understood and agreed that the Village will establish a "Health Care Task Force" comprised of <br />appropriate representatives of the Village administration and Village employees, including <br />representatives of the Union. The Task Force will be expected to study the problems of health care and <br />health care cost containment and then formulate recommendations which will be considered by the <br />Village. <br />Section 29.2. The maximum monthly premium expense assumed per employee by the Village for the <br />above medical plans shall be as follows: <br />For the Calendar Year <br />Employee <br />Employee & Spouse <br />Employee & Children <br />Family <br />2003 2004 2005 <br />$340 $364 $389 <br />$790 $845 $904 <br />$635 $679 $727 <br />$1,140 $1,220 $1,305 <br />Any Employee contribution is waived for 2003, but any time commencing Januaryl, 2004, the cost of the <br />above .medical plans exceeds the forgoing maximum limits, the excess costs will be borne by the <br />employee up to a maximum of $50 per month. <br />The Village, for the term of this agreement, will continue to provide prescription drug coverage with <br />an eight dollar ($8.00) deductible for Generic, a fifteen dollar ($15.00) deductible for Name Brand <br />and atwenty-five dollar ($25.00) deductible for Formulary. <br />Section 29.3 Any permanent full-time employee who is otherwise eligible to be covered under a <br />contract of hospitalization and medical coverage but voluntarily withdraws from the above-referenced <br />insurance coverage shall be paid the amount equivalent to 40% of the premium paid by the Village for <br />the particular coverage for each year the employee is employed by Mayfield Village but does not <br />request coverage (e.g., if the annual premium for medical coverage and dental coverage is $10,000.00 <br />and $2,000.00, respectively, an employee who waives both hospitalization and dental shall receive <br />$4,000.00 plus $800.00 in lieu of hospitalization and dental coverage for the year in question). The <br />payment authorized herein shall be made in equal quarterly installments. This payment will not be <br />made to any person who is otherwise covered under the Mayfield Village policy through a Village <br />employed spouse or other family member. (Effective in 1994). If an employee is eligible under the <br />terms of this paragraph but has worked less than one full year, the benefit described herein will be <br />prorated on a monthly basis beginning the first month in which the employee would have received the <br />benefit under the plan provided by the Village. <br />Page 27 of 35 <br />
