<br />of the calendar year the equivalent of five (5) days, or five (5) tours in the case of fire fighters, pay without having
<br />any days deducted from accumulated but unused sick leave. Sick leave accumulation lawfully transferred from
<br />prior public employees shall be included for purposes of this section. If under the foregoing circumstances, the
<br />employee has used one (1) earned sick day, or tour, during the calendar year, then said employee shall be
<br />compensated at the end of the year for four (4) days, or tours, pay without having any of said days deducted from
<br />accumulated but unused sick leave. If, under the foregoing circumstances, the employee has used two (2) earned
<br />sick days, or tours, during the calendar year, then said employee shall be compensated at the end of the year for
<br />three (3) days, or tours, pay without having any of said days deducted from accumulated but unused sick leave. If,
<br />under the foregoing circumstances, the employee has used four (4) earned sick days, our tours, during the calendar
<br />year, then said employee shall be compensated at the end of the year for one (1) day, or tour, pay without having
<br />any of said days deducted from accumulated but unused sick leave.
<br />3. Upon retirement, all full-time employees shall be entitled to receive credit and payment for accumulated but
<br />unused sick leave not to exceed two hundred forty (240) hours, provided that they have been employed by the
<br />Village for ten (10) years or more. For purposes of this Section, an employee shall be considered to have retired
<br />when such employee takes action that is inconsistent with any reasonable intent to continue employment with the
<br />Municipality.
<br />4. (a) If a full-time employee is scheduled to work on a holiday enumerated in Section 6(a), and cannot work due
<br />to illness, that day shall be considered a holiday day off and shall not be counted or compensated as a sick day.
<br />(b) "Hours of service" as used in this section, includes overtime, vacations, holidays, sick leave and all other
<br />approved and paid leaves of absent from service. "Hours of service" does not include the additional compensation
<br />time allowed an employee who works a holiday or has their normal day off on a holiday.
<br />I. Bereavement Leave
<br />In the event of death in the employee's immediate family, he shall be entitled to three (3) days off, or in the case
<br />of a member of the Fire Department under the rank of Chief, one (1) twenty-four (24);hour shift. The "immediate
<br />family" shall include the spouse, children, parents, brothers, sisters, spouse's parents and grandparents. Should an
<br />exception to the aforementioned apply, such exception to be authorized by the Mayor. An employees use of this
<br />leave while on vacation, holiday, or sick leave, shall result in the employee not being charged for such vacation,
<br />holiday or sick leave.
<br />J. Hospitalization and Medical Coverage
<br />1. The Municipality shall pay the premiums for all permanent full-time employees insured under a contract of
<br />hospitalization and medical coverage selected by Mayfield Village.
<br />2. Permanent full-time employees of the Municipality not presently insured and new permanent full-time
<br />employees of the Municipality may join the plan at such intervals as may be authorized by the Municipality's
<br />medical insurance carrier, subject to the approval of the Mayor and Finance Director.
<br />3. Any permanent full-time employee who is otherwise eligible to be covered under a contract of hospitalization
<br />and medical coverage but voluntarily withdraws from the above-referenced insurance coverage shall be paid the
<br />amount of One Thousand Dollars ($1,000.00) for each year the employee is employed by Mayfield Village but
<br />does not request coverage. For the time period covered by this ordinance, dental coverage will not be considered
<br />as part of the hospitalization or medical coverage described above. The payment authorized herein shall be made
<br />in equal quarterly installments. This payment will not be made to any person who is otherwise covered under the
<br />Mayfield Village policy through a Village employed spouse or other family member. (Effective in 1994). If an
<br />employee is eligible under the terms of this paragraph but has worked less than one full year, the benefit
