? E. The entry supervisor will terminate entry and cancel the entry pernut when: <br />1. The entry operations covered by the entry permit have been completed; or <br />2. A condition that is not allowed under the entry pernut arises in or near the permit <br />. space. <br />F. ' The Superintendent or his/her designee at each facility will retain each cancelled entry <br />permit for at least one (1) year to facilitate the review of the pernut. The review will be <br />done annually. Any problems encountered during an entry operations shall be noted on <br />the pertinent permit so that appropriate revisions to the pernut space program can be <br />made. <br />V. Entry Permit <br />A. The Individual who authorizes an entry assumes direct charge of the entry for its <br />duration. An individual who intends to authorize entry in a permit space shall, in <br />addition to the checidist items required in Section III, inc(ude in the permit, at a <br />minimum, the following information: <br />_ 1. The identify of the pennit space; <br />2. The purpose of the entry; <br />3. The date of the entry and the authorized duration; <br />4. A list of the authorized entrants entering confined space; <br />5. A list of attendants currently serving on this entry; <br />6. The individuals, by name, currently serving as entry supervisor, with a space for <br />the signature or initials of the entry supervisor who originally authorized entry; <br />7. The hazards of the permit space to be entered; <br />8. The measures used to isolate the permit space and to eliminate or control permit <br />space hazards before entry; <br />9. The acceptable entry conditions; <br />10. The results of initial periodic tests performed, accompanied by the names or <br />initials of the testers and by an indication of when the test was performed; <br />11. The rescue and emergency services that can be summoned and the means for <br />summoning those services. <br />T4.
