12. The communication procedures used by attendants and authorized entrants to <br />maintain contact during the entry; <br />13. The equipment to be provided and used during confined space entry and exit; <br />and <br />14. Any other information t insure employee safety. <br />VI. Training <br />A. It is the policy of the-M?- training will be provided so that all employees whose <br />work is regulated by this section acquire the understanding, knowledge and skills <br />necessary for the safe performance while working in confined spaces. <br />B. Training will be provided to each affected employee: <br />1. Before the empioyee is first assigned duties to enter confined spaces; <br />2. Before there is a change in assigned duties in regards to confined space <br />operations; <br />3. Whenever there is a change in pernut space operations that presents a hazard <br />about which an employee has not been previously been trained; <br />4. Whenever the District has reason to believe that there are deviations from the <br />permit space entry operations required under the Permit Required Space <br />Prog,ram - IlI, or that there are inadequacies in the employee's knowIedge of <br />equipment or procedure. <br />C. The training will establish employee proficiency in the duties required for confined <br />space operations. <br />D. It is the policy of the v11a*at each respective department certify that training required <br />for confined space procedures has been accomplished. The certification will contain <br />each employee's name, the signature or initials of the trainers and the dates of training. <br />The certification will be available for inspection by the employee and the authorized <br />representative. <br />E. Any non-employee shall receive training equivalent to that described in this Section <br />VI prior to entry. <br />15.
