D. Self-Rescue: It is the policy of the?illaq?hat the authorized entrant exit the pernut <br />space, unless it is physically impossible to do so, when: <br />1. The atteadant or entrance supecvisor orders evacuation. <br />2. An automatic alacm is activated. <br />3. The authorized entrants perceive they are idangerge and/or detect a prohibited <br />condition. <br />VIII. Training and Duties of Attendant <br />A It is the policy of the Vlllc'gthat an attendant is stationed and remains outside the permit <br />space(s) at all times during entry operation. <br />B. Individuals who work as attendants shall receive the appropriate training. (Refer to III <br />Pernut Program.) <br />C. Individuals who work as attendants shall perform their duties under the entry permit <br />system. <br />1. Number of entrants. The entry supervisor shall.ensure that attendants <br />continuously maintain an accurate count of all persons in the space. <br />2. Hazard Recognition. The entry supervisor shall ensure the attendants know of <br />and can recognize potential permit space hazards, monitor activities inside and <br />outside the permit space to determine if it is safe for entrants to remain in the <br />space. <br />3. Communication. The entry supervisor shall ensure that attendants: <br />a. Maintain effective and continuous contact with authorized entrants during <br />entry. <br />b. Order authorized entrants to evacuate the permit space immediatelv_ when: <br />1) The attendant observes a condition which is not allowed in entry perniit. <br />2) The attendant detects behavioral effects of hazard exposure. <br />3) The attendant detects a situation outside the space which could endanger <br />the entrants. <br />17.
