Agreement betrveen Mayfield Vi[lage and F.O.P. Lodge 57 <br />ARTICLE 25 <br />UliTIFOdZ1Vl ALLOWANC'E <br />Section 25.1. All newly hired probationary employees shall receive, at the Employer's expense, all <br />items as listed on the Schedule on file with the Police Chief and previously approved by the Mayor <br />and Council. There shall not be additional uniform allowance during the first year of employment <br />for newly hired employees. These items shall be paid by the Finance Department upon submittal of <br />receipts signed and approved by the Police Chief. All items purchased shall be surrendered to the <br />Employer if the employee fails to complete the probationary period. <br />Section 25.2. In 2006, all non-probationary employees shall receive an annual uniform purchase <br />and maintenance allowance in the amount of Seven Hundred Seventy-Five Dollars ($775.00). In <br />2007, the amount shall be Eight Hundred Dollars ($800.00). In 2008 the amount shall be Eight <br />Hundred Twenty-Five Dollars ($825.00). In 2009, the amount shall be Eight Hundred Fifty <br />Dollars ($850.00) and in 2010, the amount will be Eight Hundred Seventy Five Dollars ($875.00). <br />Section 25.3. Whenever different or additional uniform items are required due to promotion of a <br />full-time member of the Police Department, the Employer shall purchase those items for which bills <br />are presented together with an approval for payment from the Police Chief. <br />Section 25.4. Effective with the ratification of this contract, any employee who has a Village <br />purchased ballistic vest which is older than five (5) years old may be eligible to be reimbursed up to <br />an amount mutually agreed upon by the Village and the F.O.P. for replacement cost of a new vest. <br />However, should the officer receiving the vest terminate employment with the Village within two <br />years after receiving the vest, he will return the vest and the Village will refund the officer his or her <br />contribution. <br />Section 25.5 In the event an employee voluntarily leaves his employment prior to December <br />31 st of any year, his uniform allowance for that year shall be prorated pursuant to the following <br />formula: <br />Unifortn Allowance multiplied by Number of Days equals Allowance Due <br />365 of the year Passed <br />The prorata uniform allowance will be paid on the reimbursement basis per Section 25.2 and will <br />be paid with the following uniform allowance check run. <br />6/1/2006 Page 22 of 36
