M.V. ORDINANCE 91- <br />Page 15 <br />year. Such compensation shall be paid without having any hours deducted <br />from accumulated but unused sick leave. <br />Any permanent fifty-three (53) hour employee who, at the end of the <br />calendar year, has an accumulation of 540 hours of unused sick leave and <br />who during the same calendar year, has not used more than 53 hours of sick <br />leave during that calendar year, shall be compensated the pay equivalent <br />for fifty-three (53) hours less the hours of sick leave used during the <br />calendar year. Such compensation shall also be paid without having any <br />hours deducted from accumulated but unused sick leave. <br />For purposes of this Section, such payment shall be made at the time <br />of the first regular payroll in February at the rate of compensation in <br />effect on December 31st of the previous year for such employee. <br />J. Bereavement Leave <br />In the event of death in the employee's immediate family, he shall be <br />entitled to three (3) days off, or in the case of a member of the Fire <br />Department under the rank o£ Chief, one (1) twenty-four (24) hour shift. <br />The "immediate family" shall include the spouse, children, parents, <br />brothers, sisters, spouse's parents and grandparents. Should an exception <br />to the aforementioned apply, such exception to be authorized by the Mayor. <br />This leave shall not be charged to any other leave provision provided by <br />the Ordinance. <br />K. Hospitalization and Medical Coverage <br />1. The Municipality shall pay the premiums for all permanent <br />full-time employees insured under a contract of hospitalization and medical <br />coverage selected by Mayfield Village. <br />2. Permanent full-time employees of the Municipality not presently <br />insured and new permanent full-time employees of the Municipality may join <br />the plan at such intervals as may be authorized by the Municipality's <br />medical insurance carrier, subject to the approval of the Mayor and Finance <br />Director. <br />3. Any permanent full-time employee who is otherwise eligible to be <br />covered under a contract of hospitalization and medical coverage but <br />voluntarily withdraws from the above-referenced insurance coverage shall be <br />paid the amount of One Thousand Dollars ($1,000.00) for each year the <br />employee is employed by Mayfield Village but does not request coverage. <br />The payment authorized herein shall be made in equal quarterly <br />installments.
