The proof of insurance <br />rrzust establish that the Surface Owner and Permit <br />Holder are is insuced by a solvent insurer, licensed to <br />do business in the State of Ohio, with ]imits of five <br />#htee million dollars ($35,000,000) per occurrence for <br />pexsonal injury and propeity daniage. The Viliage <br />shall be named as an additional insured on the policy. <br />The amount of such insurance shall be reviewed bv <br />the BuildingLCommissioner everv five (S) years and <br />maYbe adjusted based on inflationarv incieases. The <br />Village shall be named as an additional insured on the <br />olic . <br />(h) (g) Emergency contact information for all <br />persons engaged in drilling and opez•ating the well <br />shall be £'iled tivith #lie Building Commissioner and <br />Fire Chief, prior to the cotnmencement of tllilling. <br />Such infoimation shall be immediately updated upon <br />any change of circumstance(s.) <br />(h) Drilling operations and site preparatian and <br />restoxation work shall complv with Chapter 1129 <br />Erosion and Sediment Control of the Codified <br />Ordinances of Mavfield Villa?e. <br />(i) Within thirty (30) days after drilling operations <br />have been comple#ed, the Pelmit Holder shall do all of <br />the following:
