Agreement between Mayfield Vfllage and F.O.P. Lodge 57 <br />Section 26.5. Any unused holiday time accrued during the calendar year and not taken by <br />December 31 St, sha11 be taken in pay. Such pay is to be made in the first pay period of tte following <br />January. <br />Section 26.6 In the event an employee voluntarily leaves his employment prior to <br />December 318t of any year, his holidaq time for that year shall be prorated pursuant to the <br />following formula: <br />Accrued Holida.vs x Number of Days of the = Vacation Days <br />365 Year Already Passed Earned <br />Tn the evest an anployee has already taken more hofiday time than that to which he is entitled at the <br />time he gives notice that he is voluntarily terminating his employment, Mayfield Village shall <br />have the right to withhold all amounts otherwise owed the employee to the extent <br />necessary to reimburse Mayfield Village. <br />ARTICLE 27 <br />FUNERAL LEAVE <br />Section 27.1. All employees shall be granted time off with pay for the purposes of attending the <br />funeral of a member of the employee's immediate family. The employee(s) shall be entitled to a <br />maximum of three (3) days off for each death in the immediate family, which days shall not be deducted <br />from his sick lea.ve credits. An additional two (2) da.ys of lea.ve could be granted for out-of-state deaths <br />or on approval from the Chief of Police and Mayor. <br />Section 27.2. In the event the death occurs during the employee's work day, he shall be granted the <br />remaining portion of the da.y off with pay. Such time shall not be deducted from any of the employee's <br />sick lea.ve credits. <br />Section 27.3. Immediate "family" shall be defined as only including the employee's spouse, children, <br />pazents, parents-in-law, brothers, sisters, grandparents, brothers-in-law and sisters-in-law. <br />Section 27.4. The use of this leave is a privilege which must be specifically requested through the <br />Chief of Police. <br />Page 23 of 35
