Agreement between Mayfield Village and F.O.P. Lodge 57 <br />Section 27.5. In cases where more time off is desired than was granted, the employee shall request in <br />advance, the use of his accrued vacation, sick time, holidays, compensatory time and/or sick/personal <br />time as credits for such additional time. <br />ARTICLE 28 <br />INSURANCES <br />Section 28.1. During the term of this Agreement, the Village shall maintain substantially similar <br />levels of inedical and dental service benefits including prescription riders and life insurance. Any <br />decision by the V?illage to change carriers or the medical and dental service benefits shall first be <br />discussed on a"meet and confer" basis with the F.O.P. before implementa.tion. <br />However, should there be proposed an actual reduction in the benefit level of inedical and/or <br />dental services (e.g. elimination of a medical service or protocol or a reduction in the number of <br />annual dental procedures) and agreement cannot be reached, either party may request expedited <br />impasse arbitration on the following basis. Each party shall select one representative who in turn <br />will select a third person and the three shall hear and decide the issue[s] within thirty (30) days of <br />the proposed change. Any decision made sha11 be subject to the Ohio Arbitration Act, O.R.C. <br />2711. If reductions or changes in the benefit plan come about as a result of the V'il.lage's inability <br />to secure previous benefit levels through it broker and or agent, such reductions or changes will <br />not be subject to the expedited impasse procedure mentioned herein, provided that such inability <br />to secure a previous benefit level is stated by the health care provider or its agent and such refusal <br />is directly verifiable by the F.O.P. <br />It is understood and agreed that the V'illage will establish a"Health Care Task Force" comprised of <br />appropriate representatives of the V'illage administration and V'illage employees, including <br />representatives of the F.O.P. The Task Force will be expected to study the problems of health care and <br />health care cost containment and then formulate recommendations which will be considered by the <br />V'illage. <br />Section 28.2. 1'he maximum monthly premium expense assumed per employee by the V'illage for the <br />above medical plans shall be as follows: <br />For the Calendar Year 2003 2004 2005 <br />Employee $340 $364 $389 <br />Employee & Spouse $790 $845 $904 <br />Employee & Children $635 $679 $727 <br />Family $1,140 $1,220 $1,305 <br />Page 24 of 35
