Agreement between Mayfield Y'rllage and F.O.P. Lodge 57 <br />Any Employee contribution is waived for 2003, but any time commencing Januazyl, 2004, the cost of <br />the above medical plans exceeds the forgoing maximum limits, the excess costs will be bome by the <br />employee up to a maximum of $50 per month. <br />The V'?llage, for the term of tlus agreement, will continue to provide prescription drug coverage <br />with an eight dollar ($8.00) deductible for Generic, a fifteen dollar ($15.00) deductible for Name <br />Brand and a twenty-five dollar ($25.00) deductible for Formulary. <br />ARTICLE 29 <br />INJURYLEAVE <br />Section 29.1. When an employee becomes injured or disabled as a result of an event arising out of <br />and in the cowse of bona fide police work as determined by the V?illage, so as to be physically unfit for <br />duty, the employee shall be granted a special leave of absence with pay by the V'illage beginning with <br />the first working day of such disability. In order to be eligible for the special on the job injury leave, as <br />provided in the article, the employee's disability must be evidenced by a certificate of a physician who <br />examined the employee. Special on the job injury leave shall not be granted to employees who incur <br />injuries of a routine na.ture or to employees while in the employment of another person or entity. <br />Section 29.2. Special on the job injury leave sha11 not exceed Nmety (90) consecutive calendar days <br />after the beginning of the lea.ve unless an extension is granted by the Employer. Said leave may be <br />canceled at an earlier time as provided below: <br />A When the employee is released by his physician to return to work; <br />B. At such time that the employee is declazed capable of performing lus nonrW duties by a <br />physician appointed by the Employer. <br />C. X prior to release for normal duties, it is determined by a physician that the employee is <br />capable of performing limited work assignments, the employee shall immediately report <br />for duty under the conditions set forth in the physiaan's certificate; <br />D. Any limited assignments of duties shall be reviewed ea.ch thiriy (30) calendar days to <br />detemune if the employees is capable of resuming normal, unlimited duties. <br />Section 29.3. The Employer shall have the right to require the employee to have a physical exam by a <br />physician appointed by the Employer resulting in the physician's certification that the employee is <br />unable to work due to the injury as a condition precedent to the employee receiving any benefits under <br />this Article. The designated physician's opinion shall govern whether the employee is actually disabled <br />or not, but sha11 not govern whether the injury was duty related. <br />Page 25 of 35
