Agreemeitt between Mayfield V'rAage and F.O.P. Lodge 57 <br />ARTICLE 30 <br />SICKLEAVE <br />Section 30.1. All permanent full-time employees shall be entitled for each complete eighty (80) hours <br />of service to sick leave of four and six tenths (4-6/10) hours with pay not to exceed one hundred <br />twenty (120) hours. Employees may use sick lea.ve with the approval of their department head for <br />absence due to illness, injury, exposure to contagious disease and illness in the immediate family. The <br />head of each department shall require their respective employees to furnish a written signed certificate <br />to justify the use of any sick lea.ve taken. At the discretion of the department head, or anytime an <br />absence due to illness excceds three (3) da.ys, a statement certifying the nature of the illness from a <br />licensed physician shall be required to justify the use of sick leave. Falsification of either a written <br />statement or a physician's certificate shall be grounds for disciplinary action including dismissal. No sick <br />leave ma.y be granted upon or after retirement or termination of employment. <br />Section 30.2. Any pennanent full-time employee who, at the end of their first full calendar year of <br />service to Mayfield V'illage, has an accumulation of forty-five (45) days, unused sick leave and who, <br />during the same calendar year, has not used any of the sick days earned during that year shall be <br />compensated at the end of the calendar year the equivalerrt of five (5) days pay without having any days <br />deducted from accumulated but unused sick leave. Sick lea.ve accumulation lawfizlly transfeired from <br />prior pubic employees shall be included for purposes of this section. If under the foregoing <br />circumstances, the employee has used one (1) eamed sick day, during the calendar year, then said <br />employee shall be compensated at the end of the yea,r for four (4) days pay without having any of said <br />days deducted from accumulated but unused sick leave. K'under the foregoing circumstances, the <br />employee has used two (2) eamed sick days, during the calendar year, then said employee sha,ll be <br />compensated at the end of the year for three (3) days, pay without having any of said days deducted <br />from accumulated but unused sick leave. IC under the foregoing circumstances, the employee has used <br />three (3) eamed da,ys, during the calendar year, then said employee shall be compensated at the end of <br />the year for two (2) days pay without having any said da.ys deducted from accumulated but unused sick <br />leave. 9 under the foregoing circumstances, the employee has used four (4) earned days, during the <br />calendar year, then said employee shall be compensated at the end of the year for one (1) da.y, pay <br />without having any of said days deducted from accumulated but unused sick leave. <br />Section 30.3. Upon retirement, all full-time employees shall be entitled to receive credit and payment <br />for accumulated but unused sick leave not to exceed nine hundred sixty (960) hours, provided that they <br />have been employed by the V?illage for ten (10) years or more. For purposes of this Section, an <br />employee shall be considered to have retired when such employee takes actions that is inconsistent with <br />any reasonable intent to continue employment with the Municipality. <br />Section 30.4. If a full-time employee is scheduled to work on a holiday enumerated in Section 26, <br />and cannot work due to illness, that da.y shall be considered a holiday da.y off and shall not be counted <br />or compensated as a sick day. <br />Page 26 of 35
