Immediate "family" shall be defined as only <br />including the employee's spouse, children, parents, parents-in-law, <br />brothers, sisters, grandparents, brothers-in-law and sisters-in- <br />law. <br />Section 27.4. The use of this Leave is a privilege that must be <br />specifically requested through the Chief of Police. <br />Section 27.5. In cases where more time off is desired than was <br />granted, the employee shall request in advance, the use of his <br />accrued vacation, sick time, holidays, compensatory time and/or <br />sick/personal time as credits for such additional time. <br />AR7~Cd.E 2& <br />INSU1?ANC~S <br />Section 28.1. During the term of this agreement, the Village will <br />maintain the level of benefits as are currently provided for <br />emplcyees in their medical and dental group insurance plan, <br />including prescription riders and life insurance. <br />Any decision by the Village to change carriers or the benefit plan <br />will be discussed with the F.O.P. before implementation. However, <br />if there is any reduction in the benefit level, there must be a <br />mutual agreement between the Village and the F.O.P. before <br />implementation. <br />It is understood and agreed that the Village will establish a <br />"Health Care Task Force comprised of appropriate representatives <br />of the Village administration and Village employees, including <br />representatives of the F.O.P. The Task Force will be expected to <br />study the problems of health care and health care cost containment <br />and then formulate recommendations which will be considered by the <br />Village. <br />Section 28.2. The maximum mr~nthly premium expense per employee to <br />the Village for the above medical plans shall be as follows: <br />For the calendar year 2000 $914.00 <br />For the calendar year 2001 $987.00 <br />For the calendar year 2002 $1,065.00 <br />If at any time during the term of this agreement, the cost of the <br />above medical plans exceeds the forgoing maximum limits, the excess <br />costs will be borne by the employee. <br />Page 31 of 43 <br />
