Collective Bargaining Agreement between <br />11layfield Village and the I.A.F.F. Loca12619 <br />_ equal to 6% of the premium. Employees selecting the 2000/4000 Medical Mutual Plan will pay, <br />'~ through payroll deduction, a contribution equal to 9% of the premium. All employees receiving <br />- . dental benefits will pay, through payroll deduction, a contribution equal to 9% of the premium <br />for the dental plan. <br />Section 29.4. Any permanent full-time non-probationary employee, who is otherwise eligible <br />to be covered under a contract of hospitalization and medical coverage but voluntarily withdraws <br />from the above-referenced insurance coverage, shall be paid Three Hundred Dollars ($300.00) <br />per month in lieu of family coverage and One Hundred Dollars ($100.00) per month in lieu of <br />single coverage. These payments shall be made in equal quarterly installments. These payments <br />will not be made to any person who is otherwise covered under the Employer's policy through a <br />spouse or other family member. No payment can be made under this section until the employee <br />provides proof to the Finance Director that he or she is covered under another health insurance <br />policy. <br />Section 29.5. The Employer will provide and pay the full premium on a $20,000 term life <br />insurance policy for each permanent full-time employee. <br />ARTICLE 30 <br />SHIFT EXCHANGE <br />Section 30.1. Employees shall have the right to exchange shifts with the Fire Chiefls <br />permission, when the change does not interfere with the operation of the Fire Department and <br />the number of paramedics on duty does not fall below three (3) and the resulting shift exchange <br />does not result in the payment by the Village of overtime to any employees. Exchanges maybe <br />for the entire twenty-four (24) hour shift, or any part thereof, in minimum increments of one (1) <br />hour. <br />Section 30.2. All requests for shift exchanges must be submitted in writing to the Fire Chief no <br />later than twenty-four (24) hours in advance of the requested exchange. All requests must be <br />signed by the person requesting the shift exchange and also include the name of the person <br />responsible for covering the hours exchanged. <br />ARTICLE 31 <br />APPLICATION <br />Section 31.1. The Employer agrees that the provisions of this Agreement, along with all work <br />rules and other appropriate regulations, will be administered on a fair and non-discriminatory <br />basis and that such rules or regulations shall not be inconsistent with this Agreement. <br />ARTICLE 32 <br />OBLIGATION TO NEGOTIATE <br />Section 32.1.
