f. Any required processing fee must accompany the application and be in the form of a <br />cashier's check or money order for the exact amount, made payable to the City of North <br />Olmsted. Those persons unable to pay the processing fee must submit a written request in <br />the form of a notarized affidavit for a waiver of the processing fee to the Secretary of the <br />Commission along with the application. This affidavit shall contain a detailed income and <br />expense history for the prior 12 months and clearly explain why applicant cannot pay the <br />processing fee. Upon receipt of the aforementioned affidavit, the Commission shall review <br />the same for completeness. Any affidavit that is not complete in detail, shall be returned to <br />the applicant along with a letter explaining the reason it is being returned. Affidavits that <br />are complete in detail, shall be reviewed by the Commission and either approved or denied <br />by a majority vote of Commission members at the next Commission meeting following <br />receipt of the affidavit. Once, the Commission acts upon the fee waiver request, the <br />Secretary of the Commission shall contact the applicant by telephone and advise him/her <br />verbally that his or her fee waiver request was either approved or denied by the <br />Commission. Applicants denied his/her fee waiver request shall be further advised that <br />he/she will be required to pay and submit all required fees to the Commission Secretary <br />within the time designated by the Commission Applicants who fail to timely submit <br />payment if required, shall not be permitted to take the examination. The Commission <br />Secretary shall record the time and date an applicant seeking a waiver was contacted by <br />telephone. Following the telephone contact with applicants, the Commission Secretary shall <br />send a letter to each applicant confirming and memorializing the telephone conversation. <br />RULE VII <br />ELIGIBLE LIST <br />Section 1: Composition: <br />The Commission shall prepare and keep open for public inspection an eligible Est which <br />shall consist of the names and examination grades of all applicants who received a passing <br />grade on an examination. The list shall rank the applicants in order of their grade and any <br />credits added thereto. The list shall be considered a public record and shall be prepared as <br />soon as is practicable following the examination. <br />Section 2: Criteria: <br />Additional criteria may be adopted by the Commission to determine which applicants shall <br />be placed on, or subsequently removed from, the eligible list. <br />Section 3: Evaluation of Additional Criteria: <br />a. Unless otherwise determined by the Civil Service Commission, the additional qualifications <br />such as the physical agility test requirement for police and fire department applicants shall <br />be evaluated using a pass/fail basis. <br />b. Unless otherwise determined by the Civil Service Commission, the additional proficiency <br />evaluation qualifications such as the typing, word processing and data entry proficiency <br />requirement for clerical (clerk -typist, secretary) applicants shall be evaluated on a pass/fail <br />basis. <br />05-24-2004 Update <br />13 <br />
