Bodily Injury $300,000/$500,000 <br />Property Damage $100,000 <br />c) Workers Compensation Statutory Limits <br />This coverage will be for each occurrence which may arise out of or result from the vendor's or <br />supplier's operations under the contract, whether such operations be by himself or by any sub- <br />contractor or anyone directly or indirectly employed by either while on the premises of the City <br />of North Olmsted during the delivery or performance of the contracted services. <br />Certificates of Insurance shall be provided to the City prior to commencing work. The City of <br />North Olmsted shall be listed as an "additional insured" re: Generai Liability coverage. <br />Ail policies and certificates must contain an endorsement that the policy cannot be canceled or <br />amended until ten (30) days after the City receives written notice by registered mail. <br />T. AFFIDAVIT AND CERTIFICATION <br />Each vendor, who is a foreign corporation, that is, a corporation not chartered in Ohio but <br />licensed to do business in Ohio, is required to submit with his proposal an affidavit duly <br />executed by the president, vice-president, or general manager of the corporation, stating in <br />said affidavit that said foreign corporation has, in accordance with the provisions of the <br />Revised Code of the State of Ohio, obtained a certificate authorizing it to do business in the <br />State of Ohio. These certificates or certified copies of them are obtainable from the Office of <br />the Secretary of State, Columbus, Ohio. <br />AA. AFFIRMATIVE ACTION AND EQUAL EMPLOYEMENT <br />The successful vendor, upon request, will be expected to furnish satisfactory proof to the City <br />that they are in full compliance with all appiicable Affirmative Action/Equal Opportunity Laws, <br />whether Federal, State or of the City of North Olmsted. <br />AB. INDEMNIFICATION AND HOLD HARMLESS <br />The successful vendor, upon request, will be expected to save the City of North Olmsted <br />harmless from any and all damages, loss or liability of any kind whatsoever occasioned upon <br />the premises, or premise adjacent thereto, by reason of any injury to person or property <br />occasioned by any act or omission, neglect or wrong of the vendor or any of its officers, <br />representatives, guests, employees, invites or other persons associated with the vendor, and <br />shall at its own costs and expense defend and protect the City of North Olmsted against any <br />and all such claims or demands. <br />11 / 12/0414:08 <br />C:\W INDOWS\TEMPWORCSE- I .DOC
