the first year's uniform maintenance aliowance upon appointment to the department, <br />and one-half (112) after completion of six (6) months of service. <br />ARTICLE 28: <br />HOSPITALIZATION <br />SECTION I: The Employer shall provide either individual or family medical insurance <br />coverage at the employee's option for each full-time employee. The Employer shall <br />have the option of seeking alternate health insurance providers or plans and will <br />attempt to offer at least two (2) different plans. Basic hospitalization will be consistent <br />with and similar to those plans offered on July 1, 1994. Basic hospitalization will be that <br />offered to all other employees of the City of North Olmsted. The Employer has the right <br />to negotiate co-payment and prescription drug rates with providers to lower the cost to <br />the Employer as well as the employee contribution. <br />SECTION 2: <br />(A) Effective July 1, 2001 and through the duration of this agreement, full-time <br />employee(s) participating within either an individual or family plan shall <br />contribute an amount equal to thirteen (13%) percent of the plan premium or <br />expected cost of such medical plan and the Employer shall contribute eighty- <br />seven (87%) percent of such cost. The employee contribution shall be withheld <br />via payroll deduction not later than the first pay period each month. <br />SECTION 3: In conjunction with participation within a medical plan, the full-time <br />employee may subscribe to and participate in the following optional health benefit <br />coverage(s): <br />Dental <br />2. Vision - Optional Plan # I(Vision plan which was in effect August 1994.) <br />Vision - Optional Plan #2 (Vision plan similar to that offered through Union <br />Eye Care) <br />3. Dental and Vision (Optional Plan #I or #2) <br />Full-time employees participating and subscribing to the optional health benefits <br />listed above within this section shall contribute an amount equal to that which is listed <br />within Section 2, (A) of this Article. Employees shall be required to participate in the <br />medical plan to be permitted to participate within the optional benefit coverage listed <br />within this Section. The employee contribution shall be withheld as a payroll deduction <br />not later than the first pay period of each month. <br />SECTION 4: Expected cost or premium is defined as the cost of all medical, hospital, <br />24 <br />. «?»?• ? ?• <br />I
