14.5 Sick Leave may be taken for any of the following reasons: <br />A. Illness or injury to the employee. <br />B. Illness or injury of a member of the Employee's immediate family. <br />C. Medical, dental, or optical examination or treatment of the Employee, where the treatment <br />may not be scheduled during non-working hours. <br />D. Exposure to contagious disease where quarantined by the Board of Health or communicable <br />to other Employees. <br />E. Pregnancy of the Employee where complications exist or childbirth by Employee's spouse. <br />14.6 The Employer shall require an Employee to fumish a standard form identifying the reason for the <br />use of sick leave as per 14.5 and signed by the Employee. <br />14.7 If inedical attention is required, the Employee shall be required to furnish a statement from a <br />licensed physician or psychotogist notifying the Employer that the Employse was unable to <br />perform his duties. Any firefighter who is sick or disabled for more than two (2) consecutive tours <br />of duty or forty (40) hour Employee who is sick or disabled for five (5) consecutive working days <br />may be required to secure and submit a physician's release certifying that they are fit to return to <br />work. This reiease must be submitted to the Emplayee's Officer in Charge before the Employee <br />wilt be permitted to rehirtt to work. <br />14.8 Employees shall be permitted to contribute sick leave hours to a sick leave bank for use by any <br />Employee who has exhausted atl of his own sick leave under the following condirions: <br />A. The Sick Leave bank shall have a maxunum balance of fifteen hundred (1500) hours. <br />B. Employees that have suffered any illness ar injary in the scope of their employment and have <br />exhausted the benefits of Article XVI or were not covered by the provisions of Article XVI <br />may draw frnm the sick leave bank after their own sick leave bank has been eachausted. <br />C. Employees that have suffered any illness or injury to themselves ar their immediate family <br />outside of the scope of their emptoyment may draw from the sick leave bank only aRer the <br />Employee has exhausted all of their own sick leave bank, vacation and holidays. <br />D. Any Employee that contributes sick leave hours to the sick leave bank shall not lose any sick <br />leave bonus as granted by section 14.2. <br />E. No Employee shali be permitted to eontribute more thea one hundred (100) hours during his <br />term of employment. <br />F. Employees who have used hours from the sick leave bank shall be required to repay the sick <br />leave bank 1 hour for every 2 hours used. Employees shail be required to repay a minimum of <br />2 hours pes pay period. Should an employee not be able to repay the sick leave bank prior to <br />27
